Remove the biohazard <br />signs from locations that are no longer used for medical waste. <br />3. Biohazard labels were observed on a tray containing clean instruments in the Scope Process Room of Endoscopy (see <br />PHOTO 12). Provide EHD with policy/ procedures for this practice. If there is no policy/ procedure, then refrain from this practice. <br />4. The facility is autoclaving solid waste. <br />5. Pathology waste is not generated at this facility. <br />6. An updated Medical Waste Management Plan was provided to the EHD. <br />7. Stericycle is the medical waste hauler for the facility. Sharps waste, pharmaceutical and chemotherapy waste is picked up by <br />the hauler. <br />8. Operator shall update page 3, section 7 of the San-i-Pak Waste Management Plan and provide a copy to the EHD. <br />9. Spore tests are done on a weekly basis. <br />10. Tracking records were reviewed during the inspection. The treatment records were not available for review. Operator shall <br />have tracking and treatment records on file, moving forward. <br />11. The autoclave treatment record indicated the autoclave ran continuously for approximately 16 hours on September 3, 2025 <br />for one cycle.
