Phase II Sampling and Analysis Plan SOP-3—Sample Control and Documentation <br /> Port of Stockton-Rail and Bridge,Stockton,CA FN <br /> • Date samples were collected <br /> • Type of sampling conducted (composite/grab) <br /> • Sample identification number <br /> • Number and type of containers used <br /> • Turn-around time <br /> • Signature of the HDR person relinquishing samples to a non-HDR person (such as a FedEx <br /> agent), with the date and time of transfer noted, and the cooler designation <br /> 5.2.4.2 Shipping Sample Cooler to Laboratory via FedEx <br /> The original COC will be placed in a plastic bag, taped to the underside of the cooler lid, and the <br /> cooler closed. The cooler will be tightly bound with filament tape. Finally, custody seals will be <br /> signed by the individual relinquishing custody and affixed in such a way that the cooler or carton <br /> cannot be opened without breaking the seals. Coolers should then be shipped to the California- <br /> certified laboratory at the nearest FedEx location. Upon receiving sample coolers, personnel from <br /> the California-certified laboratory performing analyses should fill out the "relinquished to" box of the <br /> original COC, and include a signature along with the date and time of reception. Laboratory <br /> personnel will provide a copy of this completed COC to the FTL. <br /> The original COC, copy of the completed COC, and airway bill or delivery note together constitute a <br /> complete record. The FTL will email a copy of the airbill and the COC to the PC, who will maintain <br /> the custody records as part of the analytical data file. <br /> Custody Seals: Custody seals (adhesive-backed seals designed to break if disturbed)will be preprinted. For <br /> groundwater and soil samples, affix custody seals on the sample shipping containers(coolers)in as many places as <br /> necessary to ensure security. <br /> 5.2.4.3 Hand-delivering Cooler to Laboratory <br /> The cooler will be tightly bound with filament tape. The cooler will then be transported to the <br /> laboratory by the FTL or other field technician, along with the original COC. Upon arriving at the <br /> laboratory, the FTL or other field technician will present the cooler and COC to appropriate <br /> laboratory personnel. Laboratory personnel should fill out the "relinquished to" box of the original <br /> COC and include a signature along with the date and time of reception. Laboratory personnel will <br /> provide a copy of this completed COC to the field team member who delivers the cooler. <br /> The original COC and copy of the completed COC together constitute a complete record. The FTL <br /> will email a copy of the completed COC to the PC, who will maintain the custody records as part of <br /> the analytical data file. <br /> 6 Data and Records Management <br /> All field forms and COCs will be scanned and copied into the project folder on the HDR network <br /> project file within one week of the field event completion. <br /> 5-4 <br />
