Port of Stockton Phase II ESA ��� <br /> Health and Safety Plan <br /> • Extent of known injuries or damage; <br /> • Level of medical attention; and <br /> • Preliminary root cause/corrective actions. <br /> The HDR team shall comply with all applicable statutory incident reporting requirements such as <br /> those to OSHA, the local and Federal police, or state and Federal environmental agencies. The PM <br /> will notify the client contacts. <br /> All incidents will be thoroughly investigated and a written report will be provided to the client. <br /> All near miss, damage, and injury/illness incidents must be reported as soon as possible using the <br /> HDR online Incident Report Form hosted on the HDR H&S portal. Contact the Office/Project Safety <br /> Coordinator to report the incident if online access is unavailable. <br /> Immediately notify the HDR Safety Director if any fatality or other catastrophe incidents <br /> (defined as in-patient hospitalization, amputation, or loss of an eye). The HDR Safety Director <br /> will report the incident to OSHA and/or other regulatory agencies as applicable. <br /> Safety Director contact information: <br /> Name: Jeff Kleinfelter <br /> Phone: (760)450-6497 <br /> Email: Jeff.Kleinfelter@hdrinc.com <br /> The PM (or TM if PM is not available) shall also ensure proper submittal of an Online Incident Report <br /> on IndustrySafe. IndustrySafe is safety management software for reporting near miss, injury/illness, <br /> environmental, and property damage incidents. Directions on how to log in and submit reports are <br /> found below. <br /> 14.2 How to Login to IndustrySafe <br /> To get started, follow these simple steps: <br /> • Go to the homepage on HDR's website <br /> • Click on the Industry Safe icon in the Connects Toolbox <br /> • Click on "Incidents" in the top banner to fill out a report. <br /> • To add a new incident, click on the plus sign in the green circle in the top left corner. <br /> • Complete the fields in the "Basic Information" section. <br /> • Click on the "Continue" button to finish and submit a report. <br /> A prompt report speeds up the process of getting you help and also benefits your fellow employees <br /> by preventing similar or more severe incidents. The PM shall complete the form and email it to the <br /> HSM within 8 hrs of the accident, injury or incident. <br /> 14.3 Recordkeeping <br /> HDR will maintain a Safety and Health Folder that will contain applicable documents from the <br /> following list: <br /> • Certification of medical and training requirements. <br /> 14 <br />
