Port of Stockton Phase II ESA ��� <br /> Health and Safety Plan <br /> properly containerized, labeled, and handled. Clean-up materials will be disposed of at an approved <br /> disposal facility. <br /> 13.0 Training <br /> Prior to initiating field operations, the HDR Site Supervisor will conduct a safety and health "kick-off' <br /> or"tailgate" meeting. At this time, pertinent HDR procedures and the HASP will be discussed in <br /> detail with special attention being given to site chemical and physical hazards, PPE, and emergency <br /> procedures. Upon completion of this meeting and briefing, all routine field personnel, including <br /> subcontractors, will be required to read and sign the acceptance sheet of this HASP. Applicable field <br /> forms/documents can be found in Attachment A. <br /> The HDR Site Supervisor will maintain copies of the certifications that all HDR personnel have <br /> satisfied the minimum training requirements. Supporting documentation and certificates will remain <br /> on file with the HDR PM. Field work will not be allowed to take place in the absence of adequate <br /> documentation. <br /> Applicable field forms/documentation can be found in Attachment A. <br /> Required training includes: <br /> - Wildfire training <br /> - Valley Fever <br /> - Heat Stress <br /> - ERail Safety Training <br /> - 40-Hour HAZWOPER <br /> - HDR Roadway Worker Protection (annual) <br /> - BNSF Contractor Orientation Safety with RWP (annual) <br /> 14.0Incidents, Reports, and Recordkeeping <br /> 14.1 Reporting an Incident <br /> All accidents will be reported to the PM as soon as possible, but no more than 8-hours following the <br /> accident. HDR will thoroughly investigate the accident and submit the findings of the investigation <br /> along with appropriate corrective actions to the client contact as soon as possible, but no later than <br /> one working day following the accident. <br /> All employees and subcontractors' employees shall immediately report any incident, including near <br /> misses, in which they are involved or witness to the Site Supervisor. These include spills, potential <br /> environmental permit compliance, or any environmental situation that could result in a notice of <br /> violation from a governing agency. The Site Supervisor shall immediately report the following <br /> information to the PM by phone and e-mail: <br /> • Project name and location; <br /> • Date and time of incident; <br /> • Description of incident; <br /> 13 <br />
