The evacuation plans are not included in the updated Emergency Response <br />Plan submitted to CERS. Updated the Emergency Response Plan on CERs. <br />• Violation #13 — Open. The training plan submitted does not cover evacuation procedures. <br />Uploaded the Emergency Response Training on CERs and attached the Hazcom training lesson <br />plan. <br />• Violation #14— Open. HMBP training must include the facility's emergency response contingency <br />plan. The plan submitted to CERS does not cover evacuation procedures (see violation #13). <br />Please submit training records demonstrating that employees have been provided HMBP training <br />including the content of an adequate emergency response contingency plan that covers <br />evacuation procedures. Fire and Emergency Preparedness Training records are attached. <br />Hazardous Waste: <br />• Violation #2— Open. Proof of hazardous waste training has not been submitted. Training <br />certificate is attached. <br />• Violation #11 —Open. <br />o Observation 1 - Supporting documentation demonstrating that a complete hazardous <br />waste determination has been made for each waste stream has not been submitted. <br />Analytical results for the Filter cakes and shaker screen are attached. The remaining waste <br />streams will be tested prior to our annual cleanout. <br />o Observation 2—This has not been addressed. The drum was emptied, triple rinsed and <br />recycled. <br />o Observation 3—This has not been addressed. The area around the tank was cleaned and <br />the tank will be repaired in the next six weeks. <br />• Violation #21 — Open. Disposal records have not been submitted. Disposal manifest is attached. <br />• Violation #23 —Open. The photo submitted of the used oil drum is missing the accumulation start <br />date. The used oil drum was disposed of. <br />
