Provide information on the previous land use. Provide a <br />site map which shows properly lines, buildings, treatment or storage ponds, land <br />application areas, and surface water drainage courses within 1,000 feet of the site. <br />Do active or abandoned industrial, agricultural, domestic, or groundwater monitoring <br />wells exist on the property or the wastewater disposal area(s)? If yes, provide a map <br />showing their location(s). Include available data on the depth of groundwater and <br />information on aquifers and aquitards identified beneath the property. <br />Describe all site activities which generate wastewater. Describe any daily or seasonal <br />variations in volume and the total discharged. Describe anticipated wastewater flow <br />trends for the next ten years. Is flow anticipated to increase or decrease? Is construction <br />of a distillery planned for the site? <br />Provide a list of chemicals which are used at the site, include all chemicals used in tank, <br />bottle, or other cleaning. Describe any chemical additives that are added to the process <br />water or wastewater to adjust hardness, prevent scaling, biofouling, or other undesirable <br />tendency. <br />Provide a description of the process supply wastewater; at a minimum, the process supply <br />water should be characterized for the following analytical parameters: nitrate, total <br />kjeldahl nitrogen, total dissolved solids, and pH. <br />Describe procedures to control contaminants in the wastewater streams and in the <br />wastewater ponds. Describe opportunities to reduce, reuse, or recycle wastewater. Also <br />describe the effect on wastewater quality of recycling activities. <br />Provide a flow diagram of the wastewater generation and treatment system. Include a <br />map showing the locations of all treatment equipment, clarifiers, sumps, or pump <br />stations. <br />Describe the existing wastewater disposal system. Clearly show that this method of <br />disposal will not impact the quality of the underlying groundwater. <br />Describe the storage/disposal of all stonnwater and domestic wastewater. If septic <br />systems are used for disposal of domestic wastewater, include descriptions of the septic <br />systems and the maintenance of the systems. The descriptions shall include location, <br />flow rate, size, and construction materials, as well as leachfield size and location. <br />Provide diagrams of the ponds and descriptions of the location, size and depth, <br />construction materials, inlet and outlets, depth to groundwater below the bottom of the <br />pond, and existing or planned groundwater monitoring wells. Have the wastewater ponds <br />ever overflowed? <br />Describe the quality of the wastewater. The description should include maximum and <br />average concentrations of BOD, suspended matter, nitrate and total kjeldahl nitrogen, <br />total dissolved solids (fixed and volatile), and pH. Describe seasonal variations. If <br />analytical data is not currently available, please sample the wastewater ponds and have <br />the samples analyzed by a licensed laboratory. <br />ADD1\ NAL INFORMA TION REQUIREM.
