CP26 - UST FULL Inspection Report <br />Date: <br />June 30, 2026 <br />Facility Address: <br />351 N BECKMAN RD, LODIVAN DE POL ENT INC/PACIFIC PRIDE <br />Facility Name: <br />Environmental Health Department <br />36 23 CCR 16 2631(b) and (c), and 2643(e) 2031018 - Initial and annual training provided to employees and at least one <br />trained employee is present <br />OBSERVATION: Owner/Operator failed to do one or more of the following: Provide initial training before the individual <br />performs the duties of a facility employee; Train facility employees in the proper operation and maintenance of the <br />underground storage tank system at least once every 12 months; Have at least one employee present during operating <br />hours that has been trained in the proper operation and maintenance of the UST system by a designated operator (DO). <br />REGULATION GUIDANCE: Failure to comply with one or more of the following: <br />1) Provide initial training before the individual performs the duties of a facility employee. <br />2) Train facility employees in the proper operation and maintenance of the underground storage tank system at least <br />once every 12 months in accordance with 2631(c). <br />3) Have at least one facility employee with current training on site during all operating hours. <br />CORRECTIVE ACTION: Ensure that employees receive initial and annual training in the proper operation and <br />maintenance of the UST system and that at least one employee is present during operating hours that has been trained <br />in the proper operation and maintenance of the UST system by a designated operator (DO). <br />This is a (Minor) Violation. <br />37 23 CCR 16 2613(c)(2), 2631(e) 2011006 - Copies of Facility Employee Training Certificates maintained on site or readily <br />available location <br />OBSERVATION: Owner/Operator did not maintain a copy of the "Facility Employee Training Certificate" on site or off <br />site at a readily available location. <br />REGULATION GUIDANCE: Failure to maintain on site or off site at a readily available location, if approved by the UPA, a <br />copy of the "Facility Employee Training Certificate." <br />CORRECTIVE ACTION: Maintain copies of the "Facility Employee Training Certificate" on site or off site at a readily <br />available location. <br />This is a (Minor) Violation. <br />38 23 CCR 16 2613(c)(10) and (13) 2011003 - Records of release detection monitoring and maintenance properly <br />maintained for 36 months (TCR 9d) <br />OBSERVATION: Owner/Operator failed to maintain release detection monitoring and maintenance records for 36 <br />months. <br />The following maintenance records were not on site: <br />- Monitoring System Certification Test Results for tests done on 6/23/2023, 6/25/2024 and 6/24/2025 <br />- Spill Container Test Results for tests done on 6/23/2023, 6/25/2024, 7/17/2024 (repair) and 6/24/2025 <br />- Overfill Prevention Equipment Testing Test Results for tests done on 11/13/2023 (repair) and 1/15/2024 <br />- Secondary Containment Testing Test Results for test done on 8/10/2023 <br />REGULATION GUIDANCE: Failure to maintain release detection and maintenance records for 36 months. <br />CORRECTIVE ACTION: Maintain release detection monitoring and maintenance records for at least 36 months. <br />This is a Repeat (Class 2) Violation. <br />1868 E.
