The ER plan submitted on 2/8/2022 is <br />missing the phone number of the local Unified Program Agency (UPA) on line C1. The location of the evacuation meeting <br />area is not listed on line E3. The ER plan submitted on 1/16/2023 and 1/18/2023 are both missing the first page of <br />emergency response and contingency plan. Upload a copy of the update first page of the emergency response and <br />contingency plan. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release <br />or threatened release of hazardous materials, including, but not limited to, the following: - immediate notification of <br />local emergency personnel and the Environmental Health Department (EHD) - procedures for the mitigation of a <br />release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br />evacuation plans and procedures, including immediate notice, for the site. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Repeat (Class 2) Violation. <br />Overall Inspection Comments <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were 3 violations found during the <br />inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br />by 8/29/2025. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign.
