The name of the nearest <br />medical facility and the phone number were not provided on line C4 and C5. Add the name the nearest medical facility <br />and phone number to line C4 and C5 on ER plan. <br />REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release <br />or threatened release of hazardous materials, including, but not limited to, the following: - immediate notification of <br />local emergency personnel and the Environmental Health Department (EHD) - procedures for the mitigation of a <br />release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br />evacuation plans and procedures, including immediate notice, for the site. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Repeat (Class 2) Violation. <br />Overall Inspection Comments <br />An inspection report was provided to the facility operator on the day of inspection 7/22/2025. The report was <br />amended on 8/20/2025 and replaces the initial report issued on 7/22/2025.The wrong inspection report was emailed <br />to the business with 1 violations in an error. The amended inspection report has 3 violations and was emailed to <br />management@rnbpallets.com The corrections due date has been changed from 7/24/2025 to 9/19/2025. <br />Conducted a Hazardous Materials Business Plan (HMBP) inspection. There was 3 violations found during the <br />inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br />by 9/19/2025. <br />To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br />having them sign.
