Buehler <br />December 28, 2020 <br />Page: <br />3/4 <br />resulting borehole will be backfilled with bentonite grout to approximately 6-inches below ground surface. <br />The surface will be restored to nearly match the existing surface. Arcadis will oversee the well destruction <br />activities on-site. California Department of Water Resources (DWR) Well Completion Reports (DWR Form <br />188) will be submitted upon completion of well destruction activities. <br />Soil Vapor Probe Destruction <br />Soil vapor probes will be destroyed in accordance with the SJCDEH permit. Probe locations will be <br />destroyed using an air knife and/or hand auger. All annular materials as well as the soil vapor probe and <br />associated tubing will be removed. The resulting borehole will be backfilled with neat cement grout. <br />Arcadis will oversee the soil vapor probe destruction activities on-site. California DWR Forms will also be <br />prepared and submitted for the probes. <br />Waste Management <br />Waste materials generated during well destruction activities, including soil cuttings and well debris, will be <br />placed in Department of Transportation (DOT)-approved 55-gallon drums and temporarily stored at the <br />Site pending profiling. Once profiled, the drums will be transported off-Site for disposal. Copies of the <br />waste manifests will be provided with the well destruction report. <br />Reporting <br />Upon completion of the work, Arcadis will prepare a monitoring well and soil vapor probe destruction <br />report documenting the destruction activities. The report will include a table of well details and destruction <br />date, a figure showing destroyed well locations, and the DWR forms. <br />Schedule <br />Arcadis will initiate and complete well deconstruction pre-field and field activities upon receipts of <br />necessary permits and approvals.
