GENERAL NOTES <br />1. 0.17Hot Mix Asphalt Overlay must be placed in one (1) layer with Geosynthetic <br />Pavement Interlayer (Paving Fabric) placed on existing or prepared surface. <br />2. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix Asphalt <br />used in Pavement Repair and Earthwork (Roadway Widening) has been in place for <br />a minimum of 18 hours. <br />3. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix Asphalt <br />used for Mainline Paving has been completed. <br />4. Existing trees, shrubs, fences, mailboxes, utility poles and other frontage <br />improvements must remain in place unless otherwise noted. <br />5. All earthwork, except for final shoulder grading, must be completed prior to placing <br />the final layer of Hot Mix Asphalt Pavement. No Shoulder Backing material will be <br />deposited or drifted on the final layer of Hot Mix Asphalt surfacing. Where the <br />shoulder placement exceeds 0.30' in thickness, the Shoulder Backing must be <br />placed in two (2) or more fully compacted layers. <br />6. Shoulder Backing material at all intersection radii must receive Penetration <br />Treatment at 0.25 gallon/square yard. The Contractor will be responsible for adequately scheduling inspection and testing <br />of all facilities constructed under this contract with a minimum 24 hour notice to <br />the County. All testing must conform to the Standard Specifications. All re—testing <br />will be paid by the Contractor. <br />8. The Contractor must exercise due caution and carefully preserve survey bench <br />marks, control points, reference points and all survey stakes, and must bear all <br />expenses for replacement and/or errors caused by their unnecessary loss or <br />disturbance. <br />9. Prior to commencing any work, it will be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor must notify members of the Underground Service Alert (USA) 48 hours <br />in advance of performing excavation work and/or sign relocation work by calling <br />the toll—free number 1-800-227-2600 or 811. <br />10. <br />The Contractor must field verify all existing highway and utility facilities' locations <br />and depths. It will be the Contractor's sole responsibility to locate and protect all <br />existing highway and utility facilities, so that no damage results to them during the <br />performance of this contract. The Contractor will be required to cooperate with <br />other contractors and utility companies installing new structures, utilities, and <br />services to the project.
