GENERAL NOTES 1. 0.17Hot Mix Asphalt Overlay must be placed in one (1) layer with Geosynthetic Pavement Interlayer (Paving Fabric) placed on existing or prepared surface. 2. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix Asphalt used in Pavement Repair and Earthwork (Roadway Widening) has been in place for a minimum of 18 hours. 3. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix Asphalt used for Mainline Paving has been completed. 4. Existing trees, shrubs, fences, mailboxes, utility poles and other frontage improvements must remain in place unless otherwise noted. 5. All earthwork, except for final shoulder grading, must be completed prior to placing the final layer of Hot Mix Asphalt Pavement. No Shoulder Backing material will be deposited or drifted on the final layer of Hot Mix Asphalt surfacing. Where the shoulder placement exceeds 0.30' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted layers. 6. Shoulder Backing material at all intersection radii must receive Penetration Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be confined to intersection radii plus fifty feet (50') in each direction or to the intersection conform. The Contractor will be responsible for adequately scheduling inspection and testing of all facilities constructed under this contract with a minimum 24 hour notice to the County. All testing must conform to the Standard Specifications. All re—testing will be paid by the Contractor. 8. The Contractor must exercise due caution and carefully preserve survey bench marks, control points, reference points and all survey stakes, and must bear all expenses for replacement and/or errors caused by their unnecessary loss or disturbance. 9. Prior to commencing any work, it will be the Contractor's responsibility to have each utility company locate in the field their main service lines and laterals. The Contractor must notify members of the Underground Service Alert (USA) 48 hours in advance of performing excavation work and/or sign relocation work by calling the toll—free number 1-800-227-2600 or 811. 10. The Contractor must field verify all existing highway and utility facilities' locations and depths. It will be the Contractor's sole responsibility to locate and protect all existing highway and utility facilities, so that no damage results to them during the performance of this contract. The Contractor will be required to cooperate with other contractors and utility companies installing new structures, utilities, and services to the project.
