THE CONTRACTOR'S ATTENTION IS DIRECTED TO SECTION 1540 (A)(1) OF THE CONSTRUCTION SAFETY <br />ORDERS (TITLE 8 CALIFORNIA ADMINISTRATION CODE SECTION 1540), ISSUED BY THE OCCUPATIONAL <br />SAFETY AND HEALTH STANDARDS BOARD PURSUANT TO THE CALIFORNIA OCCUPATIONAL SAFETY <br />AND HEALTH ACT OF 1973, AS AMENDED, WHICH STATES PRIOR TO OPENING AN EXCAVATION, <br />EFFORT SHALL BE MADE TO DETERMINE WHETHER UNDERGROUND INSTALLATION: I.E., SEWER, <br />WATER, FUEL, ELECTRIC LINES, ETC., WILL BE ENCOUNTERED AND, IF SO, WHERE SUCH <br />UNDERGROUND INSTALLATIONS ARE LOCATED. WHEN THE EXCAVATION APPROACHES THE <br />APPROXIMATE LOCATION OF SUCH INSTALLATION, THE EXACT LOCATION SHALL BE DETERMINED BY <br />CAREFUL PROBING AND/OR DIGGING, AND WHEN IT IS UNCOVERED. ADEQUATE PROTECTION SHALL <br />BE PROVIDED FOR THE EXISTING INSTALLATION ALL KNOWN OWNERS OF UNDERGROUND FACILITIES <br />IN THE AREA CONCERNED SHALL BE ADVISED OF PROPOSED WORK AT LEAST 48 HOURS PRIOR TO <br />THE START OF ACTUAL EXCAVATION. <br />6. THE CONTRACTOR SHALL PROVIDE SHORING, BRACING. SLOPING, OR PROVISIONS TO PROTECT <br />WORKERS FOR ALL AREAS TO BE EXCAVATED TO A DEPTH OF 5 FEET OR MORE. SAID PROTECTION TO <br />BE DONE IN ACCORDANCE WITH THE REQUIREMENTS OF LOCAL, STATE, AND FEDERAL REGULATIONS. <br />7. DURING CONSTRUCTION THE CONTRACTOR SHALL MAINTAIN THE INTEGRITY OF EXISTING FACILITIES. <br />8 CONTRACTOR SHALL BE RESPONSIBLE FOR ALL PERMITS AND LICENSES REQUIRED FOR THE <br />CONSTRUCTION AND COMPLETION OF THE PROJECT. <br />9 ALL WORK IN THE PUBLIC RIGHT-OF-WAY IS SUBJECT TO THE APPROVAL AND ACCEPTANCE OF SAN <br />JOAQUIN COUNTY PUBLIC WORKS DEPARTMENT. <br />10. CONTRACTOR SHALL COORDINATE THE REMOVAL AND/OR RELOCATION OF ALL EXISTING UTILITIES IF <br />NEEDED WITH RESPECTIVE UTILITY COMPANIES. <br />11. THE CONTRACTOR SHALL ADJUST TO GRADE ALL FACILITIES (INCLUDING, BUT NOT LIMITED TO VALVE <br />BOXES.. PULL BOXES, MAINTENANCE HOLE RIM AND COVERS, METER BOXES) WITHIN THE LIMITS OF <br />WORK AND/OR AS DIRECTED BY THE ENGINEER. THE FACILITIES SHALL BE ADJUSTED AS SET FORTH IN <br />SJCO STANDARD DRAWING NO. CW-R36 AND/OR AS DIRECTED BY ENGINEER, <br />12 THE CONTRACTOR SHALL PROVIDE A TRAFFIC CONTROL PLAN (TCP), INCLUDING ALL LIGHTS, <br />BARRICADES, SIGNS, FLAGMEN OR OTHER DEVICES NECESSARY FOR PUBLIC SAFETY. THE TCP SHALL <br />BE SUBMITTED TO AND DEEMED ACCEPTABLE BY THE COUNTY PRIOR TO START OF WORK. <br />13. ALL STATIONS REFER TO DISTANCES ALONG STREET CENTERLINE. PERPENDICULAR TO OR RADIALLY <br />OPPOSITE CENTERLINE STATIONS, UNLESS OTHERWISE NOTED. <br />14. CONTRACTOR SHALL DEMOLISH, EXCAVATE, REMOVE AND DISPOSE OF ANY EXISTING CONCRETE OR <br />ASPHALT CONCRETE PAVING, CONCRETE CURB, GUTTER AND SIDEWALK AND DELETERIOUS <br />MATERIAL AS REQUIRED TO CONSTRUCT THE CONTRACT WORK. ALL EXCESS MATERIAL GENERATED <br />BY DEMOLITION, EXCAVATION.. AND GRADING SHALL BECOME THE PROPERTY OF THE CONTRACTOR <br />AND DISPOSED OF AT THEIR OWN EXPENSE. <br />15. CONTRACTOR AGREES THAT HE SHALL ASSUME SOLE AND COMPLETE RESPONSIBILITY FOR JOBSITE <br />CONDITIONS DURING THE COURSES OF CONSTRUCTION OF THIS PROJECT, INCLUDING SAFETY OF ALL <br />PERSONS AND PROPERTY, THAT THIS REQUIREMENT SHALL APPLY CONTINUOUSLY AND NOT BE <br />LIMITED TO WORKING HOURS, <br />U S, AND THAT THE CONTRACTOR SHALL DEFEND, INDEMNIFY AND HOLD <br />THE COUNTY, OWNER, AND ENGINEER HARMLESS FROM ANY AND ALL LIABILITY, REAL OR ALLEGED.. IN <br />CONNECTION WITH THE PERFORMANCE OR WORK ON THIS PROJECT, EXCEPTING FOR THE LIABILITY <br />FROM THE SOLE NEGLIGENCE OF THE COUNTY OR ENGINEER <br />16. THE CONTRACTOR SHALL PROVIDE PUBLIC SAFETY AND TRAFFIC CONTROL IN ACCORDANCE WITH <br />THE COUNTY OF SAN JOAQUIN REQUIREMENTS. SAFE VEHICULAR AND PEDESTRIAN ACCESS SHALL <br />BE PROVIDED AT ALL TIMES DURING CONSTRUCTION. <br />17 A REGISTERED CIVIL ENGINEER OR LICENSED LAND SURVEYOR SHALL DO ALL FIELD STAKING AT THE <br />EXPENSE OF THE CONTRACTOR. <br />18 ALL DISCREPANCIES DISCOVERED BY THE CONTRACTOR IN THESE PLANS AND/OR ANY FIELD <br />CONDITIONS DISCOVERED BY THE CONTRACTOR THAT MAY DELAY OR OBSTRUCT THE PROPER <br />COMPLETION OF THE WORK PER THESE PLANS SHALL BE BROUGHT TO THE ATTENTION OF THE <br />PROJECT OWNER IMMEDIATELY UPON DISCOVERY. SAID NOTIFICATION SHALL BE IN WRITING. <br />19 CONTRACTOR IS RESPONSIBLE FOR THE PROTECTION OF ALL EXISTING MONUMENTS AND OTHER <br />SURVEY MARKERS. MONUMENTS AND SURVEY MARKERS DESTROYED DURING CONSTRUCTION SHALL <br />BE REPLACED AT THE CONTRACTOR'S EXPENSE. <br />20 THE CONTRACTOR SHALL OBTAIN AN ENCROACHMENT PERMIT FROM THE COUNTY OF SAN JOAQUIN <br />FOR ANY WORK WITHIN THE PUBLIC RIGHT OF WAY PRIOR TO CONSTRUCTION. <br />21. CONTRACTOR SHALL INDEPENDENTLY REVIEW GROUND, TOPOGRAPHY. AND EXISTING CONDITIONS <br />THROUGHOUT THE LIMITS OF WORK, AND ASSUME WHOLLY AND UNCONDITIONALLY THE RISK OF <br />COMPLETING THE WORK SET OUT ON THESE PLANS, REGARDLESS OF ROCK, WATER TABLE, OR <br />OTHER CONDITIONS WHICH THE CONTRACTOR MAY ENCOUNTER IN THE COURSE OF THE WORK. <br />22 THE CONTRACTOR SHALL CHECK AND VERIFY ALL DIMENSIONS AND EXISTING CONDITIONS BEFORE <br />LAYING OUT THE WORK AND NOTIFY THE PROJECT OWNER OF ANY DISCREPANCIES BEFORE <br />STARTING WORK. HE SHALL BE RESPONSIBLE FOR ANY ERRORS RESULTING FROM HIS FAILURE TO <br />DO SO. <br />23. DIMENSIONS TAKE PRECEDENCE OVER SCALE, DO NOT SCALE DRAWINGS. ALL DIMENSIONS ARE TO <br />FACE OF CONCRETE, FACE OF CURB, UNLESS OTHERWISE NOTED. <br />PAVING NOTES <br />1 ALL ASPHALT CONCRETE SURFACES SHALL BE SAW CUT TO A NEAT, STRAIGHT LINE AND REMOVED <br />THE EXPOSED EDGE SHALL BE SEALED WITH EMULSION PRIOR TO PAVING. THE EXPOSED BASE <br />MATERIAL SHALL BE GRADED, RECOMPACTED AND RESEALED PRIOR TO PAVING. <br />2. AFTER STRIPPING THE DEBRIS, ANY EXISTING LOOSE FILL, UNSUITABLE SOIL, SILTY SAND DEPOSITS, <br />OR DISTURBED SOILS SHALL BE EXCAVATED AND PROPERLY DISPOSED OF TO THE SATISFACTION OF <br />THE GEOTECHNICAL ENGINEER. <br />3 ASPHALT CONCRETE SHALL BE TYPE A, 3/4" MAXIMUM MEDIUM GRADING CONFORMING TO THE <br />PROVISIONS OF SECTION 39 OF THE STANDARD SPECIFICATIONS THE GRADE OF THE ASPHALT <br />BINDER TO BE MIXED WITH AGGREGATE SHALL BE GRADE PG 64-10 CONFORMING TO THE PROVISIONS <br />IN SECTION 92 OF THE STANDARD SPECIFICATIONS. <br />4. THE AGGREGATE BASE SHALL BE CLASS 2, 3/4" MAXIMUM GRADING FOR THE UPPER SIX INCHES. THE <br />DEPTH BELOW SIX INCHES MAY BE CLASS 2, 1-1/2" MAXIMUM GRADING. AGGREGATE BASE SHALL <br />CONFORM TO THE PROVISIONS OF SECTION 26 OF THE STANDARD SPECIFICATIONS. <br />5. THE SUBGRADE AND AGGREGATE BASE SHALL BE COMPACTED TO A RELATIVE COMPACTION OF 90% <br />AND 95%, RESPECTIVELY. <br />6 A MINIMUM OF ONE LANE (10') SHALL BE OPEN TO TRAFFIC AT ALL TIMES. FLAGGING SHALL BE PART <br />OF THE TRAFFIC CONTROL PLAN. <br />GRADING NOTES <br />1. ALL EARTHWORK AND SITE GRADING SHALL BE IN ACCORDANCE WITH THE RECOMMENDATIONS <br />CONTAINED IN THE GEOTECHNICAL SERVICES REPORT, TITLED PROPOSED ROAD WIDENING AND <br />RECONSTRUCTION, 30703 SOUTH MACARTHUR DRIVE, TRACY. CALIFORNIA, PREPARED BY <br />KLEINFELDER, DATED APRIL 25, 2012. <br />2. THE SITE IS GRADED TO BEST FIT WITH THE SURROUNDING CONDITIONS AND PLANNED <br />DEVELOPMENT, THE GENERAL CONTRACTOR SHALL PERFORM EARTHWORK CALCULATIONS AS <br />DEEMED NECESSARY, WHICH ACCOUNT FOR PROPOSED METHODS OF GRADING AND TRENCHING. <br />THE AMOUNT OF EARTH MOVED IS VARIABLE DEPENDENT ON, AMONG OTHER THINGS, THE GENERAL <br />CONTRACTOR'S METHODS OF OPERATION, COMPACTION, CONSOLIDATION, STRIPPING, AND UTILITY <br />TRENCHING. THE COORDINATION FOR SOIL COMPACTION TESTING SHALL BE THE RESPONSIBILITY OF THE <br />CONTRACTOR. <br />5 ALL GRADING SHALL COMPLY WITH 2O11 CALIFORNIA BUILDING CODE CHAPTER 18 AND APPENDIX <br />CHAPTER33. <br />6 NO SLOPE SHALL BE STEEPER THAN 2 HORIZONTAL TO 1 VERTICAL UNLESS OTHERWISE NOTED <br />ONTHE PLANS. <br />UTILITY NOTES <br />1 NO GUARANTEE IS INTENDED THAT UNDERGROUND OBSTRUCTIONS, NOT SHOWN ON THE PLANS. STORM DRAIN INLETS SHALL BE IN ACCORDANCE WITH THE DETAILS SHOWN ON THE PLANS. <br />MISCELLANOUS NOTES <br />1 THE CONTRACTOR SHALL LAYOUT IMPROVEMENTS FROM THE DIMENSIONS SHOWN ON THE PLANS. <br />ANY CLARIFICATION ON CONFLICTS, DISCREPANCIES OR AMBIGUITIES SHOULD BE DIRECTED TO THE <br />ENGINEER PRIOR TO CONSTRUCTION OF THESE IMPROVEMENTS. <br />2. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MATCHING EXISTING SURROUNDING LANDSCAPE <br />AND OTHER IMPROVEMENTS WITH A TRANSITION IN PAVING, CURBS, GUTTERS, SIDEWALKS, GRADES <br />ETC. AND TO AVOID AND ABRUPT OR APPARENT CHANGES IN CROSS SLOPES, LOW SPOTS OR <br />HAZARDOUS CONDITIONS. <br />3. WHERE PAVEMENT IS TO BE EXTENDED OR REMOVED, EXISTING PAVEMENT SHALL BE SAW -CUT OR <br />GRIND TO A NEAT LINE PRIOR TO PLACING NEW ASPHALT CONCRETE. <br />4. THE CONTRACTOR SHALL PROVIDE AND MAINTAIN TEMPORARY SIGNS AND MARKINGS FOR ALL <br />REGULATORY TRAFFIC SIGNS REMOVED DURING THE CONSTRUCTION. <br />5. IT IS PROHIBITED TO DISCHARGE ANYTHING EXCEPT CLEAN WATER INTO THE STORM DRAIN SYSTEM. <br />6. THE CONTRACTOR SHALL BE RESPONSIBLE FOR ADEQUATELY SCHEDULING INSPECTION AND <br />TESTING OF ALL FACILITIES CONSTRUCTED UNDER THIS CONTRACT. ALL TESTING SHALL CONFORM <br />TO THE REGULATORY AGENCY'S STANDARD SPECIFICATIONS <br />7. THE CONTRACTOR SHALL MAINTAIN A NEATLY MARKED SET OF FULL-SIZED AS -BUILT RECORD <br />DRAWINGS SHOWING THE FINAL LOCATION AND LAYOUT OF ALL STRUCTURES AND OTHER <br />FACILITIES. WHERE NECESSARY, SUPPLEMENTAL DRAWINGS SHALL BE PREPARED AND SUBMITTED <br />BY THE CONTRACTOR. <br />8. PRIOR TO ACCEPTANCE OF THE PROJECT, THE CONTRACTOR SHALL DELIVER TO THE ENGINEER, <br />ONE SET OF NEATLY MARKED AS -BUILT RECORD DRAWINGS SHOWING THE INFORMATION REQUIRED <br />ABOVE. <br />9, DUST CONTROL SHALL BE PROVIDED AT ALL TIMES, AT THE CONTRACTOR'S EXPENSE, TO MINIMIZE <br />ANY DUST NUISANCE AND SHALL BE IN ACCORDANCE WITH SECTION 10 OF CALTRANS STANDARD <br />SPECIFICATIONS. <br />10. FINAL PAVEMENT WORK SHALL NOT OCCUR WITHIN THE ROAD RIGHT-OF-WAY PRIOR TO <br />COMPLETION OF UTILITY RELOCATION WITHOUT SPECIFIC APPROVAL OF THE SJCO DIRECTOR OF <br />PUBLIC WORKS. <br />11 ANY CHANGES TO THE APPROVED IMPROVEMENT PLANS AND SPECIFICATION MUST BE PREPARED <br />AND CERTIFIED BY THE "CIVIL ENGINEER," AND SHALL BE SUBMITTED TO THE SJCO DIRECTOR OF <br />PUBLIC WORKS FOR APPROVAL PRIOR TO IMPLEMENTATION. <br />12, WHENEVER PAVEMENT IS BROKEN OR CUT IN THE INSTALLATION OF THE WORK COVERED BY THE <br />SPECIFICATIONS, THE PAVEMENT SHALL BE REPLACED AFTER PROPER BACKFILLING WITH <br />PAVEMENT MATERIAL EQUAL TO OR BETTER THAN THE MATERIALS USED IN THE ORIGINAL PAVING. <br />13 DRAINAGE FACILITIES SHALL BE MAINTAINED DURING PERIODS OF INCLEMENT WEATHER, AND <br />RESTORED TO THEIR ORIGINAL CONDITIONS UPON COMPLETION OF THE WORK. <br />14. CONTRACTOR SHALL PROTECT ALL BENCHMARKS AND SURVEYED MONUMENTS DURING <br />CONSTRUCTION. ALL AREAS DISTURBED DURING CONSTRUCTION BY GRADING, TRENCHING, OR OTHER ACTIVITIES SHALL BE <br />PROTECTED FROM EROSION DURING THE WET SEASON (OCTOBER 1 THROUGH APRIL 30). HYDROSEED IF <br />UTILIZED, MUST BE PLACED BY SEPTEMBER 15. HYDROSEED PLACED DURING THE WET SEASON SHALL BE A <br />SECONDARY EROSION PROTECTION METHOD. <br />6. SENSITIVE AREAS AND AREAS WHERE EXISTING VEGETATION IS BEING PRESERVED SHALL BE PROTECTED <br />WITH TEMPORARY FENCING (TYPE ESA). SEDIMENT CONTROL BMPs SHALL BE INSTALLED WHERE ACTIVE <br />AREAS DRAIN INTO SENSITIVE OR PRESERVED VEGETATION AREAS. <br />7 SEDIMENT CONTROL BMPs SHALL BE PLACED ALONG THE PROJECT PERIMETER WHERE DRAINAGE LEAVES <br />THE PROJECT SEDIMENT CONTROL BMPs SHALL BE MAINTAINED YEAR ROUND UNTIL THE CONSTRUCTION <br />PROJECT IS COMPLETE OR THE DRAINAGE PATTERN HAS BEEN CHANGED AND NO LONGER LEAVES THE <br />SITE. <br />8. EROSION AND SEDIMENT CONTROL MEASURES FOR THIS PROJECT SHALL COMPLY WITH THE <br />STORMWATER POLLUTION PREVENTION PLAN (SWPPP) PREPARED FOR THE PROJECT BY THE <br />CONTRACTOR IN ACCORDANCE WITH THE CALIFORNIA GENERAL CONSTRUCTION PERMIT THIS PERMIT <br />REQUIRES THAT THE SWPPP BE KEPT UP TO DATE TO REFLECT THE CHANGING SITE CONDITIONS AND THE <br />SWPPP IS TO BE AVAILABLE ON SITE AT ALL TIMES FOR FOR REVIEW BY STATE AND LOCAL INSPECTORS. <br />9. BMPs TO BE IN PLACE AND INSPECTED PRIOR TO GROUND BREAKING. <br />10. INSTALLATION AND MAINTENANCE OF EROSION CONTROL AND SEDIMENT CONTROL MEASURES ARE THE <br />RESPONSIBILITY OF THE CONTRACTOR.
