Prior to the 2017 inspection, the brace was removed and <br />a 24 diameter RC casing was placed about the full height of the 15.5 inch diameter damaged pile extension. See Photos 18 and 19 <br />dated 03/25/2019. <br />There are minor chip spalls present on the pile extensions throughout the bridge. The spalls are typically less than 6 inches in <br />diameter x 1 inch in depth, do not expose rebar and are sporadically located. The condition does not warrant a defect. See Photo 20 <br />dated 03/25/2019. <br />There are horizontal cracks up to 0.03 inches wide spaced 1 to 3 feet on center present on the RC casing of Pile Extension 4 of Bent <br />4. (See Photos 21 and 22 dated 03/25/2019). This condition has been previously reported and no significant changes were noted. <br />The lower portions of the original pile extensions in Bents 3 and 4 exhibit minor abrasion. Coarse aggregate was visible but appeared <br />sound. (See Photos 23 and 24 dated 03/25/2019). This condition has been previously reported and no significant changes were <br />noted. <br />Both abutments are protected with gabion mattresses. See Photos 2 through 5 dated 03/08/2021. <br />The monolithic wingwalls have been included in the quantity of Element 215. <br />There were no significant defects noted. <br />Both Bents 2 and 5 are protected with gabion mattresses and Rock Slope Protection (RSP). See Photos 3 and 4 dated 03/08/2021. <br />Both Bents 3 and 4 are protected by the 1/4-ton rock as detailed in the 2013 mitigation as-built plans. See Photo 4 dated 03/08/2021. <br />This condition could not be verified during this investigation due to the depth and high velocity flow of water. <br />Element 256 has been included to account for the gabion mattress and RSP on the embankments at both ends of the bridge. <br />The top row of the gabion mattresses at Abutment 6 is worn, pulled apart and filled with dirt. The condition appeared to be due to <br />human foot traffic, and does not warrant a defect of Element 256. See Photo 5 and Photo 33 dated 03/25/2019. <br />There were no significant defects noted.
