There was 6 inches of stagnant water in the creek below Cells 3 and 4
during this investigation. Piers 3 and 4 were partially submerged and inspected with the aid of probing. All visible elements were fully
inspected. A complete routine inspection was performed.
MISCELLANEOUS
A new routine map view photo was added during this investigation. See Photo 1.
SUBSTRUCTURE
The patch between the abutment and the non-monolithic wingwalls has failed at Abutment 1 on the left and at Abutment 5 on the right.
(See Photos 2 and 3). In addition, there is a spall up to 4 inches tall x 2 inches wide x 1 inch deep in the left wingwall at Abutment 5. (See
Photo 3). There is a similar spall up to 32 inches tall x 2 inches wide x 0.5 inches deep on the right side of Abutment 5. See Photo 4.
WATERWAY
The previously reported timber debris at the right side of Piers 2 and 3 has been removed or washed away. The 2008 work
recommendation to remove the debris has been updated to reflect the new condition.
There is significant sedimentation in Spans 1, 2 and 4. Span 1, which is the most severe, has approximately 6 feet of sediment. (See
Photo 1 dated 06/26/2018). This condition has been previously reported and no significant changes were noted.
INSPECTION COMMENTARY
TYPE OF MEMBRANE
TYPE OF DECK PROTECTION
DECK STRUCTURE TYPE
TYPE OF WEARING SURFACE
WEARING SURFACE / PROTECTIVE SYSTEM
1006AVERAGE DAILY TRAFFIC
2013 YEAR OF ADT
56 KMBYPASS, DETOUR LENGTH
10 %(109) TRUCK ADT %
3989FUTURE ADT
2040YEAR OF FUTURE ADT
HISTORICAL SIGNIFICANCE 5: NOT ELIGIBLE FOR NRHP
N-NOT APPLICABLE
N-NOT APPLICABLE
N-NOT APPLICABLE
N-NOT APPLICABLE
DECK STRUCTURE INFORMATION
9.8 M
9.5 M
DECK AND ROADWAY
LANES 2
55
0-NOT ON NHS
PARALLEL STRUCTURE
TEMPORARY STRUCTURE
DEFENSE HIGHWAY
FUNCTIONAL CLASS
SPEED
NATIONAL HIGHWAY SYSTEM
TOLL
DIRECTION OF TRAFFIC
DESIGNATED NATIONAL NETWORK
TYPE OF SERVICE
FEDERAL LANDS HWY
6 EQUAL MIN CRITERIAAPPR ROADWAY ALIGN
5 ABOVE TOLERABLEDECK GEOMETRY
9.5 MAPPROACH RDWY WIDTH
BRIDGE MEDIAN
LEFT 0.0 M RIGHT 0.0 MCURB OR SIDEWALK
TOTAL WIDTH
NET WIDTH
9.5 MINVENTORY ROUTE TOTAL HORIZ CLEAR
99.99 MINVENTORY ROUTE MIN VERT CLEAR
3-ON FREE ROAD
08-MINOR COLLECTOR RURAL
0-NOT STRAHNET
N-NONE EXISTS
2-2 WAY
0-NOT APPLICABLE
0-NOT ON NET
0 NO MEDIAN
1-HIGHWAY
DECK ROADWAY/OPERATIONAL INFORMATIONDECK GEOMETRY
Roadway: 0.33 ft br, 31.33 ft, 0.33 ft br
DECK CROSS SECTION
LRS INVENTORY RTE & SUBRTE
BASE HIGHWAY NETWORK 0-NOT ON NET
- NOT APPLICABLE FOR THIS BRIDGE.
0 inchesOVERLAY THICKNESS (inches)
N/A
(51)
(52)
(50)
(32)
(33)
(107)
(108)
A)
B)
C)
(29)
(30)
(19)
(114)
(115)
(37)
(42a)
(12)
(13)
(104)
(26)
(100)
(101)
(102)
(10)
(47)
(68)
(72)
(105)
(110)
(20)
(28a)
(103)
CONDITION INFORMATION
15.2 MLENGTH (49)
SPECIAL INSPECTION INFORMATION
