Department of Transportation - Division of Maintenance <br />Structure Maintenance & Investigations <br />SUBSTRUCTURE ELEMENT INSPECTION RATINGS AND NOTES (60) SUBSTRUCTURE RATING = 7 <br />Elem Defect/ Element Description Env Total Units Qty in each Condition State <br />No. Prot Defect Qty C;S 1 CS 2 CS 3 C:S 4 <br />210 Pier Wall -RC 2 8 m 7 0 1 0 <br />1080 Delamination/Spall/Patched Area 2 1 0 0 1 0 <br />(210-1080) Delamination/Spall/Patched Area <br />There are three spalls on the top of the right side of Pier 2 on the Span 1 side as follows: <br />There is a vertical spall up to 6 inches tall x 2 inches long x 1 inch deep with exposed rebar in the interior corner of the curtain wall. <br />There is a horizontal spall up to 12 inches long x 1 inch tall x 0.5 inches deep with exposed rebar in the interior face of the curtain <br />wall. <br />There is a spall up to 2 inches in diameter x 0.5 inch deep in the face of the pier wall. <br />(For all three spalls see Photo 9 dated 11/09/2022). No section loss observed in the exposed rebar. <br />These conditions have been previously reported and no significant changes were noted. <br />215 Abutment -RC 2 29 m 29 0 0 0 <br />(215) Abutment -RC <br />This quantity includes monolithic wingwalls. <br />There were no significant defects noted. <br />220 Pile Cap/Footing-RC 2 1 m 1 0 0 0 <br />(220) Pile Cap/Footing-RC <br />The Span 1 side of Pier 2 footing is exposed horizontally up 4 feet and vertically up to 8 inches on the left side. This condition is not <br />caused by stream action and therefore does not warrant a defect at this time. (See Photo 10 dated 11/09/2022). This condition has <br />been previously reported and no significant changes were noted. <br />227 Pile -RC 2 1 ea. 1 0 0 0 <br />(227) Pile -RC <br />The pile element is included to indicate the presence of piles on this structure. The piles were not exposed for visual inspection. No <br />indication of pile distress was noted in any substructure element. <br />228 Pile -Timber 2 1 ea. 1 0 0 0 <br />(228) Pile -Timber <br />The pile element is included to indicate the presence of piles on this structure. The piles were not exposed for visual inspection.
