BOX 6377 <br /> STOCKTON, CA 95207 <br /> 209-982-1804 <br /> 209-982-1803 FAX <br /> Bonnadonna's&sbcglobal.net <br /> EMERGENCY RESPONSE REPORT <br /> On Wednesday, 11 October 2006, Bonadonna's Asphalt Repairs, Inc. <br /> (BAR) received a phone call from Mr. Chuck Stone, CSP, Manager of Safety <br /> of Matheson Trucking, Inc. indicating one of their trucks had been involved <br /> in an accident near 11396 N. Highway 99, Lodi, California. <br /> Mr. Stone reported that a Matheson truck traveling north on State <br /> Highway 99. He had exited the freeway through the cyclone fence and <br /> landed on the Frontage Road on its side, wherein the diesel fuel leaked onto <br /> the asphalt on the Frontage Road and a portion ran into the irrigation ditch <br /> proximate to the Frontage Road. It was further reported that the diesel fuel <br /> ponded and remained on the road for several hours as the accident occurred <br /> at 0530. <br /> Mr. Stone further reported that he had contacted Ramos <br /> Environmental Services of West Sacramento. Project manager, Joe Ormonde <br /> was on site with the Ramos Emergency Response Team and had contained <br /> the diesel fuel and utilized absorbent materials. They had also utilized 55 <br /> gallon drums to contain initial clean up materials for proper transport and <br /> disposal. <br /> BAR was contacted at approximately 1430 hours and was told a <br /> grinder/planer was required to remove a portion of the asphalt, as the diesel <br /> fuel had penetrated for a lengthy period. It was further reported that the area <br /> removed would require patching after grinding. Mr. Stone retained BAR and <br /> it was indicated that we would require about 90 minutes to pull men from <br /> jobs, mobilize, and be on site. <br /> Jim Hoblitzell arrived at the site at 1500 hours and met Mr. Stone of <br /> Matheson Trucking and Mr. Ormonde of Ramos Environmental.
