� CE]INED <br />i <br />jAIq 14 �U 10 <br />San Joaquin County <br />Environmental Health Department <br />� I�� NMENTAL <br />T�cP, HI ME <br />1868 East Hazelton Avenue, Stockton, California AsLIC010 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: v.or /q ehd%11o Ztio <br />RCRA Large Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />1 <br />Date: <br />Lawrence Livermore National Lab - Site 300 <br />15999 W CORRAL HOLLOW RD Tracy <br />September 30 2015 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />— Remarks <br />713 <br />CCR 66273.35 Stored UW onsite greater than one year or was unable to demonstrate the time UW has been stored. <br />The following observations were made during the inspection: <br />-At 865 Corp Yard, two fluorescent tubes were stored open and unlabeled. <br />-At Corp Yard north of 865, there were electronics waste and a CRT stored on the ground and a pallet including a <br />microwave, a computer monitor, a keyboard, and lamps without a label. <br />-At building 874, electronics waste was stored on a pallet outside without a label. <br />Universal waste may be accumulated for no longer than one year from the date the universal waste was generated. <br />The universal waste handler shall be able to demonstrate the length of time that the universal waste has been <br />accumulated. Since it could not be demonstrated how long this waste was stored on site, immediately dispose of this <br />waste and submit a copy of the disposal receipt to the EHD. <br />This is a Class II violation. <br />715 <br />CCR 66273.39 Failed to keep complete records of each shipment of universal waste for three years. <br />Copies of universal waste disposal records for 2012, 2013, 2014 and 2015 were not found on site. Universal waste <br />shipments are going to the Livermore facility for disposal according to Kenneth Newman. Universal waste disposal <br />records shall be retained for three years and have them readily available for review. Immediately locate a copy of all <br />universal waste disposal receipts for the last three years, maintain them on site, and submit copies to the EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />***THIS IS THE FULL OFFICIAL INSPECTION REPORT**' <br />The inspection report was completed on October 28, 2015. This full official inspection report supersedes the <br />checklist that was left on site with John Scott on October 19, 2015. One added violation, item # 401, is included in <br />this inspection report that was not reflected on the checklist that was left on site. The inspection lasted for 5 days <br />and included the following dates: <br />September 30, 2015 <br />October 1, 2015 <br />October 5, 2015 <br />October 15, 2015 <br />October 19, 2015 <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork within 30 days of receipt of full official inspection report. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information <br />online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
