In the event an <br /> emergency occurs, it is very important that all personnel know what to do. This knowledge will help <br /> reduce the potential for bodily injury,and equipment damage. <br /> EMERGENCY-FUEL OR OIL RELEASE <br /> Chain Of Command: <br /> 1. John Stephenson 209-628-7275 <br /> 2. Rod Hanson 435-979-0923 <br /> 3. Chris Hughes 801-513-8637 <br /> Possible Emergency Scenarios: The following is a list of possible fire scenarios: <br /> 1. Spilled diesel or used oil ignites. <br /> 2. Spilled diesel or used oil releases towards surface water. <br /> 3. Spilled diesel or used oil contaminates soil or groundwater. <br /> Emergency Response Equipment Needs: In the event of fuel or oil release, sorbent materials should be <br /> employed. Although most release scenarios would remain contained in secondary containment, in the <br /> event of a containment breach,the facility's front end loader is capable of deploying fly ash as a <br /> temporary berm until the liquid can be safely removed. The largest tank is 275 gallons, so spill should be <br /> localized. <br /> Critical Plant Operations Shut Down Or Control: In the event of fuel or oil release, all associated piping <br /> and valves should be shut if safe for the employee. This included any valve on a secondary containment <br /> structure. <br /> Emergency Response Training Requirements: None. Headwaters personnel are not trained in <br /> emergency response or CPR. <br /> Personnel Evacuation and Accounting: Person will be evacuated as necessary. If the likelihood of injury <br /> or fire, all personnel will be evacuated to the Primary Assembly Area. In the event that the Primary <br /> Assembly Area is not feasible,employees will be instructed to evacuate to the Secondary Assembly Area <br /> or completely off-site. Headwaters management will contact the appropriate emergency contact for <br /> cleanup,emergency services or notification. <br />
