UNIVAR USA INC. <br />CONTINGENCY PLAN <br />06 /14/11 Replaces 0 3/10/10 <br />SECTION <br /> <br />SUBJECT PAGE 16 <br />SIX <br /> <br />EMERGENCY PROCEDURES OF 22 <br />TRANSPORTATION INCIDENTS <br />If a release of a hazardous material or waste is discovered during transportation, it is <br />incumbent upon the driver of the transport vehicle to attempt, to the maximum extent <br />practicable, to stop the release and clean-up any spilled substances. The following <br />emergency procedures apply: <br />EMERGENCY PROCEDURES: <br />1. If proper safety clothing and equipment is available and there is no risk of injury <br />to the driver, the driver should attempt to stop or reduce the release or to at <br />least contain the release. <br />2. When the release is controlled, the driver shall telephone the Branch Operations <br />Manager/Supervisor as soon as possible and report the incident. This shall <br />occur before the vehicle may be moved from the site of the release. The <br />vehicle may be moved if it is necessary to drive somewhere to find a phone. <br />3. The Branch Operations Manager/Supervisor assesses the need to: <br />Obtain outside assistance from local Hazmat teams. <br />Respond with our own personnel and equipment. <br />Notify the National Response Center and complete a DOT F5800.1. <br />4. If it is suspected that a reportable quantity (RQ) amount has been released, the <br />Branch Operations Manager/Supervisor shall contact the Regional Regulatory <br />Manager immediately. The Regional Regulatory Manager will report the spill to <br />appropriate Federal and State authorities. In the event the Regional Regulatory <br />Manager is not available, the Branch Operations Manager/Supervisor shall <br />contact the Regional Operations Manager or Corporate Operations in Redmond <br />for assistance. <br />5. If a Univar USA Branch is made aware of a transportation incident involving a <br />common carrier carrying Univar USA lading within the Branch's territory, the <br />Branch Operations Manager/Supervisor, in conjunction with the Regional <br />Regulatory Manager or Regional Operations Manager and the common carrier, <br />shall decide if it is appropriate to respond to the incident with Univar USA <br />personnel and equipment.
