Failed to show secondary containment for the four 20,000 gallon diesel AST is sufficient. The Plan <br /> does not address the issue this issue. Secondary containment must be "sufficiently impervious" for the <br /> entire secondary containment: retaining walls, dike/ berms, floors, so that discharge from primary <br /> containment system will not escape containment before cleanup occurs and that it is sufficient to hold <br /> precipitation freeboard. Correct by providing documentation in the Plan that address this information. <br /> # 24. Listing of emergency contact phone numbers incomplete and not current. Owner/ operator direct <br /> cell phone number not current. Jimco Manager's name is spilled incorrectly in several areas of the Plan. <br /> Contact listing lacks phone numbers for National Response Center, cleanup contractors with whom you <br /> have an agreement for response and all appropriate Federal, State and local agencies who must be <br /> contacted in case of discharge. Correct by revising the emergency contact list to include the missing <br /> information. <br /> # 27. Plan lacks description of potential equipment failure. Plan does not address potential equipment <br /> failure of diesel AST and any mobile/ portable container X55 gallons. Plan must indicate reasonable <br /> potential for equipment failure (such as un/loading equipment, tank overflow, rupture, leakage or any <br /> equipment known to be source of discharge), include in the Plan: a prediction of direction, flow rate, total <br /> quantity of oil which could be discharged from facility as result of each type of major equipment failure. <br /> Correct by including this description in the Plan. <br /> # 31. Failed to conduct inspections and tests. Plan lacks written procedures that the operator/owner or <br /> the certifying PE has developed. Written procedures and a record of inspections and test, signed by the <br /> appropriate supervisor/ inspector, with the SPCC Plan for a period of 3 years. Correct by including <br /> written procedures for inspections and testing. <br /> # 32. Failed to train personnel on discharge prevention, designate a person accountable for discharge <br /> prevention and schedule prevention discharge briefings annually. No documentation found on training <br /> employees. Train the oil-handling personnel in operations and maintenance of equipment to prevent <br /> discharge, discharge procedure protocols, applicable pollution control laws, rules and regulations, <br /> general facility operations and contents of SPCC Plan. A designated person must be a person who is <br /> accountable for discharge prevention and who reports to facility management.
