Failed to describe how oil handling, processing and storage areas are secured and controlled, how <br /> master flow and drain valves are secured, how unauthorized access to starter controls on oil pumps are <br /> prevented, how out of service and un/loading connections are secured, appropriateness of security <br /> lighting to both prevent acts of vandalism and assist in the discovery of oil discharges. Mone of the <br /> above information is found. Correct by providing the above missing information in the Plan. <br /> # 50. Failed to test liquid level sensors. Testing method was not addressed in the Plan. Engineer or <br /> update each container installation in accordance with good engineering practice to avoid discharge with <br /> an approved liquid level device listed in CFR 112.8(c)(8). You must regularly test liquid level sensing <br /> devices to ensure proper operations. Correct by addressing the test method and how regularly liquid <br /> level sensing devices will be tested. <br /> # 52. Failed to promptly correct visible discharges. On the turbine pumps, pipings and towards the <br /> bottom of the tanks nearby bolts, there are dark staining which indicate leakages. A visible oily drip can <br /> be seen on the aboveground product piping adjacent to the turbines. Correct by promptly removing the <br /> dark staining and oily residues which results from loss of oil from container, including but not limited to <br /> seams, gaskets, piping, pumps, valves, rivets and bolts. Dispose the waste properly according to <br /> Hazardous Waste Regulations in California Code of Regulations, Title 22. Maintain a disposal receipt <br /> onsite for a minimum of 3 years. <br /> Submit to SJC EHD within 30 days (12/30/2010): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($t 22). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Ins ector: Receir�Qy: Title: <br /> EHD 23-02-003 Rev 08110!10 CONTINUATION FORM <br />
