An inspection was last done on March 17,2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 4/17/16. This information has not been received resulting in a non-compliant status for this facility. The outstanding violations for the March 17,2016 inspection include: 1)Violation 601 -Plan that does not follow the sequence specified in this section lacks a cross-referencing section. 2)Violation 605-Plan failed to address facility layout, operations discharge prevention methods and containers. 3)Violation 612-Failed to provide secondary containment, diversionary structures,or equipment to prevent discharge. 4)Violation 618-Failed to conduct inspections or maintain records for 3 years. 5)Violation 619-Failed to train personnel on discharge prevention. 6)Violation 621 -Failed to schedule and conduct discharge prevention briefings at least annually. 7)Violation 706-Failed to provide and maintain adequate secondary containment. 8)Violation 708-Failed to maintain records of drainage from diked areas. 9)Violation 710-Failed to perform tank inspections that take into account size, configuration, and design. 10)Violation 715-Failed to regularly test liquid level sensing devices to ensure proper operation. 11)Violation 717-Failed to promptly correct visible discharges and/or remove accumulations of oil in diked areas. The outstanding violations from the March 18,2015 inspection include: 1)Violation 102-Failed to properly close a tank that is considered permanently closed. 2)Violation 201 -Plan certification by a Professional Engineer(PE)missing or incomplete. 3)Violation 301 -Failed to amend Plan as necessary. 4)Violation 303-Failed to have a PE certify technical amendments. 5)Violation 601 -Plan that does not follow the sequence specified in this section lacks a cross-referencing section. 6)Violation 602-Plan lacks approval of management to fully commit the necessary resources to implement the Plan. 7)Violation 603-No facility diagram or didn't show location and contents of containers,transfer stations,and pipes. 8)Violation 605-Plan failed to address facility layout, operations discharge prevention methods and containers. 9)Violation 611 -Plan failed to include a description of potential equipment failure or provide equivalence. 10)Violation 612-Failed to provide secondary containment,diversionary structures,or equipment to prevent discharge. 11)Violation 618-Failed to conduct inspections or maintain records for 3 years. 12)Violation 619-Failed to train personnel on discharge prevention. 13)Violation 620-Failed to designate a person accountable for discharge prevention. 14)Violation 621 -Failed to schedule and conduct discharge prevention briefings at least annually. 15)Violation 703-Failed to design facility drainage system from undiked areas to retain in the facility. 16)Violation 710-Failed to perform tank inspections that take into account size,configuration,and design. 17)Violation 715-Failed to regularly test liquid level sensing devices to ensure proper operation. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation,Class II.
