Buried piping shall be tested for integrity and leaks at the <br /> time of installation, modification, construction, relocation, or replacement. Immediately perform integrity and leak <br /> testing of this piping and ensure that piping is tested when required, or provide equivalence as allowed by CFR <br /> 112.7(a)(2). Submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> 728 CFR 112.8(d)(5) Failed to adequately warn vehicles entering facility to protect piping and other transfer operations. <br /> No warning signs were observed at several AST locations, to warn vehicles of aboveground piping, containers, or <br /> other appurtenances. The reviewed SPCC plan also indicated (Table A-1) that the following tanks had no vehicle <br /> protection and no proper signage for tank contents and hazard: <br /> 1)400-gallon diesel tank at dairy production building <br /> 2) 2,000-gallon diesel tank at PIA farm office <br /> 3)400-gallon diesel tank at fresh water booster station <br /> 4) 150-gallon diesel tank at new waste water plant <br /> A warning must be provided to all vehicles entering the facility to be sure that no vehicle will endanger aboveground <br /> piping or other oil transfer operations. Immediately provide a warning in accordance with CFR Part 112 for all <br /> vehicles entering the facility, or provide equivalence as allowed by CFR 112.7(a)(2). <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
