The <br />missing alarms include: the fuel alarm for the C3 - Satellite 2/3 dispenser pan sensor occurred on 5/6/2016. There <br />were no records of alarm logs or follow up actions for this fuel alarm. During the monthly inspection, the designated <br />operator shall review the alarm history for the previous month, check that each alarm was documented and responded <br />to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br />monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />This is a minor violation. <br />211 <br />CCR 2715(f) Designated operator employee training not performed or log not kept. <br />The designated operator employee training was not found on site. The designated operator shall train facility <br />employees for which he or she is responsible in the proper operation and maintenance of the UST system once every <br />12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />This is a minor violation. <br />315 <br />HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul 2003). <br />Liquid was observed in the STP sump . If water could enter into the secondary containment by precipitation or <br />infiltration, it must be removed and disposed of properly. The service technician removed approximately 1 cup of liquid <br />from the STP sump. Ensure that all sumps and annular spaces are maintained free of liquid. <br />This is a minor violation. <br />319 <br />CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />The diesel UST spill container failed the one hour hydrostatic test. All USTs shall be equipped with a spill container <br />that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary <br />container or provide a means to keep the container empty. <br />The technician replaced the drain valve. The diesel UST was retested and passed. <br />This was corrected on site. <br />This is a minor violation. <br />Page 5 of 6 <br />
