An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations of the liquid sensors, leak detectors, and the monitoring panel. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. The facility failed to maintain alarm logs and records of follow up actions. A fuel alarm for the C3 - Satellite 2/3 dispenser pan sensor occurred on 5/6/2016. There were not records of alarm logs or follow up actions for this fuel alarm. There was a sensor out alarm for the C1 - Dispenser 1 Satellite sensor that occurred on 4/5/2016. There were no records of follow up actions for this fuel alarm. These records should be maintained on site. Locate alarm logs and/or records of follow up actions. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. Records of the 2014 and the 2015 annual monitoring system certification test results, and the secondary containment test results for UST system were not found on site. The secondary containment test results needed are: 1) 3/17/15, for the routine secondary containment test results, 2) 4/8/16, for the secondary containment test results after repairs were made, and 3) 11/15/16 for the 6 month retest for the secondary containment test result. These records shall be kept on site for three years. Immediately locate and maintain these records on site and submit copies to the EHD. This is a minor violation. 206 CCR 2712 Failure to comply with UST operating permit conditions. For Service Request: SR0072452, the following documents have not been received: disposal manifest for removed fuel piping, and analytical results from soil sampling. The analytical results must be emailed directly from the laboratory to the EHD. Correct immediately. This is a minor violation. Page 4 of 6
