See 'Sampling Protocol for Routine Tank Removals' and 'Verification Analysis' on page <br />6 and 7 of the permit application for required analytical tests and number of samples to <br />be taken and witnessed by EHD. <br />5. In addition, the following information must be submitted within 30 days after the UST <br />removal date: <br />a. Analytical results: only results that come directly from a third party California certified <br />laboratory with affixed chain of custody will be accepted by EHD <br />b. Piping Hazardous Waste Manifest <br />c. Hazardous Waste Manifest for rinsate and residual product, if needed <br />d. Backfill Excavation Certificate, if needed <br />6. A copy of the Fire Department permit must be submitted at the time of removal. <br />7. If concrete will be broken near the tank or main (non -satellite) piping, a secondary <br />containment test must be performed on the tank, sump, or piping once concrete is <br />poured in accordance with the manufacturer's guidelines. <br />8. Secondary containment testing for UDC 1 and 2/3 and all replaced piping is required six <br />months after installation. Provide 48 hours notification to the EHD when this test is <br />scheduled. <br />9. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />(may be issued to the installation company), or a Tank Tester's license issued to the <br />individual by the State Water Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically <br />present at the work site who possesses a valid, current California UST Service <br />Technician certificate issued by the International Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />10. Individuals installing underground storage tank system components must possess or <br />work under the direct and personal supervision of an individual physically present at the <br />work site who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, <br />and <br />b. A valid, current UST Installation/Retrofitting certificate issued by the International <br />Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being installed. <br />2 <br />
