The designated operator failed to check and note the correct test dates for the secondary containment testing date performed on 3/17/2015 on the January-December 2017 designated operator monthly inspection report.The designated operator incorrectly logged in the date as 3/1/2016. During the monthly inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Mr. Mike Eliason corrected the secondary containment testing date on all the designated operator reports from January-December 2017 during the inspection. This was corrected on site.
