CLEMENTS <br />March 14, 2018 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />301 <br />CFR 112.5(a) Failed to amend Plan as necessary. <br />In the site's Spill Prevention, Control, and Countermeasure (SPCC) Plan, on section 8.3.3 Integrity Testing <br />Schedule, it mentions that the site is undergoing a site review on October 2016, and once the site review is <br />completed, that the plan will be amended with integrity testing schedule. The plan has not been amended to include <br />that information at this time. The Spill Prevention, Control, and Countermeasure (SPCC) Plan must be amended <br />when there is a change in the facility design, construction, operation, or maintenance that materially affects its <br />potential for a discharge, within 6 months of the change, and implemented as soon as possible, not later than 6 <br />months following preparation of the amendment. Immediately make all necessary amendments to the SPCC Plan <br />to accurately represent the procedures and policies currently in place at the facility. <br />This is a minor violation. <br />603 <br />CFR 112.7(a)(3) Failed to adequately describe the physical layout of the facility in the Plan. <br />The facility diagram is missing the location of the second five -thousand gallon extender oil tank located in the <br />Rubber AC area. The Spill Prevention, Control, and Countermeasure (SPCC) Plan shall include a facility diagram <br />which must mark the location and contents of each fixed storage container and the storage area where mobile or <br />portable containers are located. It must identify the location of and mark as "exempt" underground tanks. It must <br />also include all transfer stations and connecting pipes, including intra -facility gathering lines. Immediately update <br />the facility diagram to include all of the required information. Submit a legible copy of the updated facility diagram to <br />the EHD for review. <br />This is a Class II violation. <br />604 <br />CFR 112.7(a)(3)(i) Plan failed to include oil type and storage capacity for each container. <br />The Spill Prevention, Control, and Countermeasure (SPCC) Plan failed to include the second five -thousand gallon <br />extender oil tank located in the Rubber AC area. The following shall be addressed in the Spill Prevention, Control, <br />and Countermeasure (SPCC) Plan: <br />- type of oil in each fixed container and it's storage capacity. For mobile or potable containers, the type of oil and <br />storage capacity for each container or an estimate of the potential number of mobile or portable containers, the <br />types of oil, and anticipated storage capacities <br />- discharge prevention measures including procedures for routine handling of products <br />- discharge or drainage controls such as secondary containment, equipment, and procedures for the control of a <br />discharge <br />- countermeasures for discharge discovery, response, and cleanup <br />- methods of disposal of recovered materials <br />- contact list and phone numbers for the facility response coordinator, National Response Center, cleanup <br />contractors, and all appropriate Federal, State, and local agencies. <br />Immediately amend the SPCC Plan to include all required information. <br />This is a minor violation. <br />FA0010336 PR0522199 SCO01 03/14/2018 <br />EHD 28-01 Rev. 11/07/2017 Page 4 of 6 Aboveground Petroleum Storage Act OIR <br />1868 E.
