Details of the evaluation procedure if the EPA standard <br /> procedure was not used for the evaluation. If the EPA <br /> evaluation procedure was used, list any deviations or <br /> modifications to the procedure. <br /> b. Complete set of all BPA required attachment sheets. <br /> Q. Individual test logs and/or field notes. <br /> d. Details of calculations and any applicable graphs or <br /> charts generated during the evaluation. <br /> e. If applicable, a statement from the evaluator confirming <br /> that all equipment at the test site was properly <br /> maintained and calibrated to the level of accuracy <br /> necessary for a valid evaluation. <br /> 2 . A summary of the manufacturer's operating procedures for the <br /> equipment. All pages of these procedures must be dated and <br /> include a revision number. Also, a statement from the <br /> manufacturer confirming the use of the submitted procedure <br /> during the evaluation. <br /> 3 . An affidavit from the manufacturer confirming that there is no <br /> mutual financial interest between the equipment manufacturer <br /> and the third-party evaluator. <br /> 4. Complete installation and operations manual for the equipment. <br /> 5. A resume, including all applicable formal training and <br /> experience, from each of the people who conducted the <br /> evaluation. <br /> r <br /> L✓ <br />
