Kettleman Ln. Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> and the California Health and Safety Code. Submit a statement and supporting documentation <br /> explaining how this waste was managed by October 19, 2012. <br /> 301. Automotive fluids have spilled on the floor beneath the east and west lube bays. Automotive fluids <br /> have spilled on the second from the bottom shelf. This shelf is located to the north of the three step <br /> stairs west of the east lube bay. Water contaminated with automotive fluids has collected in the floor <br /> trench of the lube shop pit. Facilities shall be maintained and operated to minimize the possibility of a <br /> fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human <br /> health or the environment. Immediately clean up the spilled automotive fluids, and manage according to <br /> Title 22 hazardous waste regulations. Employee was unsure of where the floor trench in the lube shop <br /> pit led to. Submit a statement and supporting documentation explaining how this waste was managed <br /> and to where the floor trench in the lube shop pit leads to by October 19, 2012. <br /> 404. The drum of hazardous waste antifreeze was left open. All hazardous waste containers shall be <br /> kept closed at all times except when adding or removing waste. Immediately close this container and <br /> ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by October 19, 2012. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453,,, QL <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
