Inspections and tests must be <br /> conducted and stored in accordance with the written procedures developed for this facility in the SPCC Plan. <br /> Immediately begin conducting all inspections and tests in accordance with the procedures in the SPCC Plan, or <br /> amend the Plan and have it recertified by a Professional Engineer to accurately reflect the inspection procedures <br /> currently followed at the facility. <br /> This is a Class 11 violation. <br /> 14 CFR 112.7(0(1) Failed to train personnel on discharge prevention. <br /> The docuii c�ritation for oil handliiiy T u+su inui Liauuilg un tmi iau;ury Spill Prevention Control and Counter Meabi;ic <br /> Plan were not provided upon request during the inspection. At a minimum, oil handling personnel shall be trained +° <br /> the operation and maintenance of equipment to prevent discharges; discharge procedure protocols; applicable <br /> pollution control laws, rules, and regulations; general facility operations; and the contents of the Spill Prevention <br /> Control, and Countermeasure Plan. Immediately provide this training to all oil handling personnel and submit a cup, <br /> of the training log to the EHD. <br /> This is a Class 11 violation. <br /> CFR 112.8(c)(2) Diked area is not sufficiently impervious to contain discharged oil. <br /> The secondary containment wall for the tank i.: i is not impervious to contain discharged oil. The existing <br /> secondary containment has a hole approximately two inches in diameter. This hole has a cable running through it <br /> towards the rail spur which is just north of the tank farm secondary containment wall. Along the north and west walls <br /> of the tank farm secondary containment were multiple seams that had been repaired but now appear to be in <br /> disrepair. One of the repaired seams along the northern part of the wall is wide enough that you can see right <br /> through the wall to the other side.
