GUIDELINES FOR COMPLETING FORM A: MONTHLY IN-SERVICE, <br />VERTICAL, ABOVEGROUND, ATMOSPHERIC TANK INSPECTIONS <br />DIKED AREA AROUND TANK <br />1. Is the dike wall open or lowered in specific location or sections that would result in <br />product leaving the containment area before reaching a level at the highest wall <br />elevation? Are there open pipe spool penetrations that would allow product to flow out of <br />dike before reaching highest dike wall elevation? <br />_ Open / Cut Lower <br />Dike Wall <br />Tank <br />Open Pipe Spool <br />2. Dike drain should be closed and deemed fully operational to allow draining of <br />accumulated rain water. Operator to stroke valve to confirm operability and leave valve in <br />the closed position. _ <br />Tank Dike Drain Valve <br />Rain Water <br />Accumulation <br />3. Check for product near the tank chime and at all piping and valve locations inside the <br />containment area. <br />4. Are there combustible materials such as wood, cardboard, paper or significant dead <br />vegetation in the containment area? If so, have it removed. <br />5. Look for any sign of product near tank, piping or valves. <br />TANK FOUNDATION AND BOTTOM <br />6. On double bottom tanks, leak detection tell-tale ports shall be opened, visually inspected <br />and "sniffed" to determine if there is LEL present. If one of the ports has a valve on it, <br />open that one first. Once this inspection is complete, tell -tales to be capped closed. <br />::::::t <br />�— 2"d Bottom Capped Tell -Tale <br />on Double Bottom <br />7. Check for "spalling" or general degradation of concrete ring wall, if applicable. <br />Determine if ring wall degradation is exposing the underside of the tank chime. Chime coverage and standing water will support corrosion. Need to identify and correct <br />as soon as possible. <br />9.
