GUIDELINES FOR COMPLETING FORM A: MONTHLY IN-SERVICE,
VERTICAL, ABOVEGROUND, ATMOSPHERIC TANK INSPECTIONS
10. If present, grounding rod, cable, and tank lug assembly to be verified as intact.
Typical Tank
Grounding Assembly
TANK SHELL
11. Walk tank perimeter to determine if shell is deformed in any areas. This generally occurs
in the upper shell sections where the plate material is thinner.
12. "Weeping" is more prevalent in the lower tank shell sections where the hydrostatic
pressure is the greatest.
13. Visually check for broken welds or loose or missing steps.
14. Walk tank perimeter to determine if areas of shell coating failures have resulted in active
corrosion to a point where visible metal loss is observed.
Tank Shell
Heavy corrosion resulting in localized or
uniform visible metal loss
Tank Chime
APPURTENANCES
15. Hand gauge tank level and compare to auto gauging system to verify auto gauge is
properly calibrated. Reference Measurement Manual for details, but deviations greater
than Y," require adjustments.
16. Visually check conduit to level switch run to verify system is mechanically complete.
17. If tank is equipped, does the fixed roof vent contain debris that would not allow it to
vent?
INSULATION
18. Visual Inspection to determine if there are tears, holes or missing sections that would
allow water in between insulation and tank shell.
TANK FIXED (CONE) ROOF
19. Look for localized buckling that may hold water resulting in corrosion areas.
FLOATING ROOF, IF APPLICABLE
20. Visually verify roof is dry and no presence of liquids on roof deck. Only necessary to
view through one inspection hatch and only if it is accessible within handrail area.
OTHER
21. Any additional comments / concerns in regards to items not covered above.
195 OM F-37 MA D-04 Form A - Monthly In -Service Aboveground Vertical Atmospheric Tank Inspections 1 V1 4
