All registered <br />professionals must be licensed by the State of California. <br />11. Submit preapprovals with workplans and all reports with reimbursement requests to the State <br />Water Resources Control Board Cleanup Fund in a timely manner to avoid delays that would <br />result in non-compliance with this Order. <br />Quarterly Reporting <br />12. By 15 July 2000, and the 15th day of the month following each calendar quarter, submit a <br />quarterly progress report for the former Chevron Station #9-5775 site work that provides the <br />results of groundwater monitoring and sample analysis andsite work performed during the <br />previous calendar quarter. Groundwater analysis is to include total petroleum hydrocarbons as <br />gasoline, benzene, toluene, ethyl benzene, total xylenes, ethylene dibromide, 1,2-dichloroethane, <br />MtBE, TSA, D1PE, FTBE, TAME, ethanol, and methanol. The quarterly progress reports shall <br />include: <br />a. Groundwater elevation contour maps <br />b. Pollutant concentration contour maps <br />c. Groundwater purge data sheets <br />d. Tabular results of current and all previous data <br />e. Narrative describing the work presented in the report, including methodology <br />f. Discussion of the status of investigation and cleanup activities <br />g. Evaluation of the effectiveness of cleanup activities and an estimate of pollutants treated <br />versus pollutants remaining <br />h. A schedule for future activities <br />i. Include data from the last three months, document treatment or disposal of any <br />contaminated soil or groundwater generated during the investigation and cleanup <br />activities. <br />16. if groundwater monitoring indicates the groundwater plume has migrated beyond laterally or <br />vertically defined limits during the quarter, the quarterly report shall include a work plan and <br />schedule, with work to begin within thirty days of report submittal, to define the new plume <br />limits. <br />Dated: <br />5 <br />GARY M.
