s <br />Ms. Thuy Tran <br />San Joaquin County EHD <br />May 26, 2010 <br />Page 2 <br />Item #62 - Failed to file an exception report <br />Generator shall submit an exception report to DTSC if hasn't received a copy of the manifest with <br />TSDF signature within 45 days waste was accepted. When submitting an exception report, <br />Chevron would include an explanation of the steps taken while attempting to obtain the <br />appropriate documentation (i.e. phone calls to TSDF and Transporter) <br />Items #79, 83, and 84 - Failed to store Universal Waste lamps in closed, secured container in a manner to <br />prevent breakage,- Failed to label it as `Universal waste fluorescent lamps",, Unable to demonstrate length <br />of time the fluorescent lamp has been accumulated. <br />Fluorescent lamps have been placed in appropriate container and secured to prevent breakage. <br />They are properly labeled as Universal Waste with accumulation start date (3/15/2010). <br />Aboveground Petroleum Storage Act Inspection findings <br />The inventory list in the SPCC Plan lists incorrect shell capacities. Verify the shell capacity for each <br />container and update the SPCC Plan accordingly. <br />The shell capacities of all containers have been verified and corrections are reflected in the <br />facility's updated SPCC Plan. <br />Failed to conduct inspections and tests in accordance with written procedures and/or maintain signed <br />records of inspections and tests as required by the facility SPCC Plan on site for 3 years <br />Chevron has 3 tanks (#21, #22, #134) listed and labeled as Out of Service, but have not been <br />"Permanently Closed" in accordance with 40CFR 112.2. Chevron has decided to permanently <br />close these tanks by ensuring the following requirements are met: <br />(1) All liquid and sludge has been removed from each container and connecting line; and <br />(2) All connecting lines and piping have been disconnected from the container and <br />blanked off, all valves (except for ventilation valves) have been closed and locked, and <br />conspicuous signs have been posted on each container stating that it is a permanently <br />closed container and noting the date of closure. <br />Manways remain partially opened for ventilation purposes <br />If you have any questions, please contact me at one of the numbers above, <br />Sincerely, <br />(D Com-- Ute. <br />Danielle Ortega <br />9 <br />Compliance Specialist <br />Chevron Products Company <br />Cc: Mike Raasch - Terminal Myr <br />
