The SPCC Plan was certified on May 1, 2006. According to the cover sheet, the Plan was reviewed <br /> and recertified in February 2007. No record of a review or additional certification could be located. A <br /> review and evaluation of the plan must be conducted at least once every 5 years. You must document <br /> your completion of the review and evaluation, and must sign a statement as to whether you will amend <br /> the Plan, either at the beginning or end of the Plan, or in a log or appendix to the Plan. As a result of <br /> this review and evaluation, the SPCC Plan must be amended within 6 months of review, and recertified <br /> by a professional engineer if any technical amendments were made. Immediately conduct a review of <br /> the facility SPCC Plan and make any necessary amendments. <br /> 18. A facility diagram was not included with the Plan. The Plan shall include a facility diagram which <br /> must mark the location and contents of each fixed storage container and the storage area where mobile <br /> or portable containers are located. It must also include all transfer stations and connecting pipes, <br /> including intra-facility gathering lines. Immediately provide a facility diagram which includes all of the <br /> required information. Submit a legible copy of the facility diagram to the EHD by July 20, 2011. <br /> 24. The contact information for the San Joaquin County Office of Emergency Services (SJC OES) was <br /> not included in the SPCC Plan. The SPCC Plan must include a contact list and phone numbers for the <br /> facility response coordinator, National Response Center, cleanup contractors with whom you have an <br /> agreement for response, and all appropriate federal, state, and local agencies who must be contacted in <br /> case of a discharge. Immediately update the contact list to include the most current version of the <br /> required information. <br /> 32. Annual training is not being conducted for the aboveground petroleum storage program and <br /> containers. At a minimum, oil-handling personnel must be trained in the operation and maintenance of <br /> equipment to prevent discharges, discharge procedure protocols, applicable pollution control laws, rules <br /> & regulations, general facility operations, and the contents of the facility's SPCC Plan. Annual briefings <br /> must be conducted to assure adequate understanding of the SPCC Plan and highlight and describe any <br /> known discharges or failures, malfunctioning components, and any recently developed precautionary <br /> measures. Immediately conduct necessary training for all oil handling personnel. Submit copies of <br /> training records to the EHD by July 20, 2011. <br /> 48. Integrity testing has not been conducted on the motor oil and diesel tanks. The facility shall test or <br /> inspect each aboveground container for integrity on a regular schedule and whenever you make material <br /> repairs. Examples of these integrity tests include, but are not limited to: visual inspection, hydrostatic <br /> testing, radiographic testing, ultrasonic testing, acoustic emissions testing, or other systems of non- <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received Bye Title: <br /> EHD 23-02-00 R 8/10/10 CONTINUATION FORM <br />
