6.0 <br />5.5 Provide Tanknology's technician free and clear physical access to all AST(s), piping and <br />containment. <br />5.6 Ensure that all tank surfaces and welds to be inspected by visual or ultrasonic <br />examination are free from scale, rust, dirt, oil, grease and other material that may <br />interfere with these examinations. <br />5.7 Provide Tanknology's technician reasonable physical access to the top of the AST(s) for <br />the inspection of the roofs. <br />5.8 Provide Tanknology a copy of all previous monthly and annual inspection records. <br />5.9 Provide Tanknology all repair or alteration records for the AST(s). <br />5.10 Provide Tanknology all information from the AST(s) manufacturer(s), including original <br />tank design specifications and manufacturers standards. <br />Exclusions From Proposal: The Following Items Are Not Included in the Proposal: <br />6.1 Expedited testing. <br />6.2 Additional testing required upon detection of corrosion. <br />6.3 Additional work requested and agreed to outside the scope of this proposal will be billed <br />at time and material, unless otherwise agreed in writing. <br />6.4 Any required safety training. Safety training will be billed at a per man hourly rate. <br />6.5 Any state or local permits. <br />6.6 Dewatering, storage or disposal of water. <br />6.7 Repair or replacement of AST(s), conduits, materials, parts, or equipment unless <br />otherwise specifically provided for in this agreement. <br />6.8 Environmental evaluation, assessments or testing. <br />6.9 Excavating, loading, handling, disposal or backfill needed due to contaminated soil or <br />water. <br />6.10 Asphalt patching or replacement. <br />6.11 Landscaping restoration, seeding or sod. <br />6.12 Freight unless specifically noted. <br />6.13 Applicable sales tax unless specifically noted. <br />7.0 Schedule: Purchaser and Tanknology will, within 30 days after acceptance of this Proposal, develop <br />a mutually acceptable work schedule. Expedited scheduling may require an additional mobilization fee to <br />meet Purchaser's timeframe. <br />8.0 Reporting: Tanknology will provide Purchaser a final assessment report incorporating the <br />following: <br />• A general description of the methods used to perform the testing <br />• A general description of the Site and Tank layouts, including an explanation of UT data points <br />included in the report <br />• An Executive Summary, including recommendations for corrective actions <br />• Comprehensive Tank Photos <br />• Certification by STI Certified Tank Inspector <br />• Report turnaround timeframe is generally 30 days or less from the date of the inspection. Expedited <br />processing of the report may result in additional fees to the client. <br />The report will be submitted electronically to Purchaser, via email or thumb drive depending on the file size, <br />unless Tanknology is otherwise directed. <br />9.0 Access to TANCSTM. In connection with Tanknology's performance of services, certain <br />information of Purchaser may be downloaded and maintained in Tanknology's proprietary Web Reporting <br />Access ProgramTm ("TANCST"'"). Purchaser will be permitted to access and use "TANCSTM only on the <br />terms and conditions set forth in Tanknology's TANCSTm License and Non -Disclosure Agreement as it <br />may be modified by Tanknology from time to time (the "License Agreement"). Before accessing and using <br />TANCTM, Purchaser must open, read, and accept the terms of the online License Agreement. Purchaser <br />agrees to comply with the License Agreement terms and to follow Tanknology's policies, procedures, <br />requirements and instructions in connection with Purchaser's access to and use of TANCST14 <br />
