Failed to amend SPCC Plan within 6 months when there is an applicable change to the facility. An <br /> unused diesel tank, approximately 2500 gallons, located by the truck wash bay was taken out of service. <br /> A tank monitoring system panel for the 1 OK diesel tank was removed from the office approximately 5 <br /> months ago. The SPCC Plan must be amended when there is a change in the facility design, <br /> construction, operation or maintenance that materially affects its potential for a discharge. Changes <br /> include, but not limited to: reconstruction, replacement, installation of piping system; construction or <br /> demolition that might alter secondary containment structures; changes of product or service; or revision <br /> of standard operation or maintenance procedures at the facility. An amendment made under this section <br /> must be prepared within 6 months, and implemented as soon as possible, but not later than 6 months <br /> following preparation of the amendment. Correct by adding the amendment to the plan regarding the <br /> decommissioning of the 2500 gallon diesel tank, the removal of the tank monitoring system panel and <br /> any other changes to the facility. <br /> # 16. SPCC Plan lacks full approval of management at a level of authority to commit the necessary <br /> resources to implement the Plan. On page iii, this section lacks the signature and the designated named <br /> individual who has the authority to commit the necessary resources. Correct by having the designated <br /> individual sign this section. <br /> # 18. Facility diagram failed to show location and contents of each container. In the Site Plan diagram, <br /> the map lacks the location of the 250 gallon AST (Aboveground Storage Tank) hydraulic fluid in the <br /> shop/ maintenance building with the various 55 gallon portable containers of assorted weight oil: SAE <br /> 50, SAE 90, 85W-140, 15W-40. The facility diagram needs to mark the location and contents of each <br /> fixed oil storage container and the storage area where mobile/ portable containers are located. Correct <br /> by including the mark location for these contents. <br /> # 19. SPCC Plan failed to include description of product type and storage capacity of each container. <br /> The plan is missing information on the 3rd AST: hydraulic fluid located in the shop/ maintenance building <br /> and the various 55 gallon portable containers of assorted weight oil. For mobile/ portable containers, <br /> either provide the type of oil and storage capacity for each container or provide an estimate of the <br /> potential number of mobile/portable containers, type of oil, and anticipated storage capacity. Correct by <br /> documenting the missing 3r AST and the various 55 gallons portable containers. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY,RATE. <br /> EHD Inspector:THUY Received T1tleG7if�•--� <br />
