SPCC Plan failed to include contact list and phone numbers of the facility's response coordinator, <br /> cleanup contractors, and agencies to notify in case of discharge. Contact list and phone numbers for the <br /> facility response coordinator, National Response Center, cleanup contractors with whom you have an <br /> agreement for response, and all appropriate Federal, State and local agencies (example, San Joaquin <br /> County OES (Office of Emergency Services) and SJC EHD (Environmental Health Department)) who <br /> must be contacted in case of a discharge. Correct by having a contact list with the required contact <br /> phone numbers in the Plan. <br /> # 31. Failed to conduct inspection and test in accordance with written procedures and maintain signed <br /> records of inspections and test as required by the facility's SPCC Plan on-site for a minimum of 3 years. <br /> On page 14, the Plan lists that routine inspections performance: daily: on AST and fueling area; <br /> monthly.- checking container for deterioration, discharges, integrity, liquid level sensing devices; <br /> annually: leak test, monitoring system; and every 10 years: shell integrity testing. No documentation of <br /> such routine inspection performance was found. Correct by keeping a documented written record of <br /> inspections and tests, signed by the appropriate supervisor or inspection, with the SPCC Plan for a <br /> period of 3 years. <br /> # 32. Failed to train personnel on discharge prevention and schedule prevention discharge prevention <br /> briefings at least annually. No documentation of training was found. At minimum, train your oil-handling <br /> personnel in the operation and maintenance of equipment to prevent discharge,- discharge procedures <br /> protocols; applicable pollution control laws, rules and regulations; general facility operations; and the <br /> contents of the facility's SPCC Plan. Correct by training employees who handles any equipment or is <br /> involved in the use of any type of petroleum product/waste and conduct an annual refresher training. <br /> # 48. Failed to test the aboveground container for integrity on a regular schedule, On page 14, it states <br /> that every 10 years, a tank shell integrity testing is scheduled to be conducted. The diesel, use oil and <br /> new motor oil were installed in 1997. The scheduled testing should have been conducted in 2007. <br /> Correct by having a qualified personnel perform the tests and inspections following the industry <br /> standards, frequency and type of testing and inspections. Keep the inspection and test reports with the <br /> SPCC Plan for a minimum of 3 years. <br /> # 50. Failed to installed and regularly test approved liquid level sensing devices. The diesel, new and <br /> use oil have a visual gauge on the exterior of the tank. The hydraulic AST does not appear to have a <br /> liquid level sensing device. Correct by engineering or updating each container installation in accordance <br /> with good engineering practice to avoid discharges and have its liquid level sensing device tested to <br /> ensure proper operations. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector:THUY TRAN � Received Title: <br />
