It is a one page form which you would fill out the inspection date, inspector's name, <br /> facility address and the facilty's EPA ID#and that you have signed and dated the document. <br /> I understand you are trying to re-classify your facility as Tier 1. The Return to Compliance has nothing to <br /> do with the SPCC or for APSA Violation #6-59. Majority of APSA violation #6-59 does not apply to your <br /> facility and I will NOT enforce the regulations if they do not apply. <br /> "Return to Compliance Certification" is the only document I am missing. Again, this one page"Return to <br /> Compliance Certification" can be found at SJC EHD website <br /> http://www.sjgov.org/ehd/Programs/Unitlll/retcomp.pdf. I have also attached a copy in this email. This form <br /> is found on the back side of the inspection checklist form. I need this one page document to complete the <br /> file.
