San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209) 468-3433 Web:www.sjgov.org/ehd <br /> Aboveground Petroleum Storage Act Inspection Report <br /> Facility Name: Facility Address: Date: <br /> California Health Care Facility 7707 Austin Rd, Stockton September 04, 2014 <br /> The cumulative aboveground petroleum storage at this facility is 42,000 gallons (four 10,000 gallon and one 2,000 <br /> gallon above ground storage tanks containing diesel fuel utilized in operation of onsite generators). Consent to <br /> perform the inspection and take photos was given by Steve Sasson, Associate Hazardous Materials Specialist. <br /> The routine inspection began on September4, 2014 and continued on September 9, 2014 and was completed on <br /> September 23, 2014. A checklist of findings was left onsite at the time of inspection, a final report was delivered to <br /> the facility on October 9, 2014. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by November 9, 2014. <br /> Please be aware as of January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> Aboveground Petroleum Storage Act (APSA) information online to the California Environmental Reporting System <br /> (CERS) at http-)7cers.calepa.ca.gov in addition to any other relevant activities and required fields.
