Perks Petroleum Specialists, Inc. <br />765 East Greg Street, Suite 103 <br />Sparks, NV 89431 <br />Re: Love's Travel Stop <br />1553 West Colony Road <br />Ripon, CA 95366 <br />Environmental Health Department <br />Linda Turkatte, REHS, Director <br />Kasey Foley, REHS, Assistant Director <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Rodney Estrada, REHS <br />Willy Ng, REHS <br />Muniappa Naidu, REHS <br />FILE COPY <br />Amended SR0076606 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for an amended permit to include the addition of one Bravo B-834 commercial <br />single wall transition sump located on the northeast side of the existing canopy and addition of <br />one Veeder Root 794380-208 piping sump sensor, in addition to the original scope of work to <br />remove ten existing under dispenser containments (UDCs) and replace with eight new Bravo B- <br />8000 single-wall deep fiberglass UDCs, excavate and collect soil samples beneath each UDC <br />location, and install eight Gilbarco Encore dispensers that include dispensing diesel, in addition <br />to the 87- and 91 -product, on the underground storage tank (UST) system at the above noted <br />address. The amended permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Vicki McCartney at (209) 468-9852 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Witness soil sampling below the ten UDCs, below removed piping if extensive <br />beyond the UDCs, and stockpiles. <br />b. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. If it is determined that <br />a cold start is necessary, a copy of the alarm history shall be printed prior to <br />performing the "cold start" of the Veeder Root system and made available at the time <br />of EHD inspection. <br />
