The facility shall implement the SPCC plan in accordance to what the plan states. Immediately <br /> implement policies and procedures outlined in the SPCC plan. <br /> This is a minor violation. <br /> 301 CFR 112.5(a) Failed to amend Plan as necessary. <br /> In section 4.1 of the SPCC plan, the emergency contact, "Angel Gomez,"is no longer the Plant Manager.Also,the <br /> Regional Water Quality Control Board's phone number is inaccurate. <br /> In section 4.2 of the SPCC plan,the cleanup contractor's phone number is inaccurate. <br /> The Spill Prevention, Control, and Countermeasure(SPCC) Plan must be amended when there is a change in the <br /> facility design, construction, operation, or maintenance that materially affects its potential for a discharge,within 6 <br /> months of the change, and implemented as soon as possible, not later than 6 months following preparation of the <br /> amendment. Immediately make all necessary amendments to the SPCC Plan to accurately represent the procedures <br /> and policies currently in place at the facility. <br /> This is a minor violation. <br /> 302 CFR 112.5(b) Failed to review Plan once every five years and/or implement any resulting amendments. <br /> The Spill Prevention, Control, and Countermeasure(SPCC)Plan was last reviewed on Octer 15, 2010. A review and <br /> evaluation of the plan must be conducted at least once every 5 years. You must document your completion of the <br /> review and evaluation, and must sign a statement as to whether you will amend the Plan, either at the beginning or <br /> end of the Plan, or in a log or appendix to the Plan. The following words will suffice: <br /> "I have completed the review and evaluation of the SPCC Plan for(name of facility)on(date), and will(will not) <br /> amend the Plan as a result." <br /> Immediately amend the SPCC Plan to include all necessary documentation of the Plan reviews and ensure that future <br /> Plan reviews are properly documented. <br /> This is a Class II violation. <br /> Page 4 of 6 <br />
