The facility failed to implement the SPCC plan.The SPCC plan states in section 3.5.7"Service Truck,"that the service truck is housed within an earthen berm spill containment area when not fueling and servicing equipment around the facility.The service truck was stationed adjacent to the truck wash, not housed within an earthen berm spill containment area. The facility shall implement the SPCC plan in accordance to what the plan states. Immediately implement policies and procedures outlined in the SPCC plan. This is a minor violation. 301 CFR 112.5(a) Failed to amend Plan as necessary. In section 4.1 of the SPCC plan, the emergency contact, "Angel Gomez,"is no longer the Plant Manager.Also,the Regional Water Quality Control Board's phone number is inaccurate. In section 4.2 of the SPCC plan,the cleanup contractor's phone number is inaccurate. The Spill Prevention, Control, and Countermeasure(SPCC) Plan must be amended when there is a change in the facility design, construction, operation, or maintenance that materially affects its potential for a discharge,within 6 months of the change, and implemented as soon as possible, not later than 6 months following preparation of the amendment. Immediately make all necessary amendments to the SPCC Plan to accurately represent the procedures and policies currently in place at the facility. This is a minor violation. 302 CFR 112.5(b) Failed to review Plan once every five years and/or implement any resulting amendments. The Spill Prevention, Control, and Countermeasure(SPCC)Plan was last reviewed on Octer 15, 2010. A review and evaluation of the plan must be conducted at least once every 5 years. You must document your completion of the review and evaluation, and must sign a statement as to whether you will amend the Plan, either at the beginning or end of the Plan, or in a log or appendix to the Plan. The following words will suffice: "I have completed the review and evaluation of the SPCC Plan for(name of facility)on(date), and will(will not) amend the Plan as a result." Immediately amend the SPCC Plan to include all necessary documentation of the Plan reviews and ensure that future Plan reviews are properly documented. This is a Class II violation. Page 4 of 6
