The SPCC plan does not include contact information for the California Emergency Management <br /> Agency or the National Response Center. The SPCC Plan must include a contact list and phone <br /> numbers for the facility response coordinator, National Response Center, cleanup contractors with <br /> whom you have an agreement for response, and all appropriate federal, state, and local agencies who <br /> must be contacted in case of a discharge. Immediately update the contact list to include the most <br /> current version of the required information. <br /> 25. The SPCC plan does not include information and procedures to enable a person reporting a <br /> discharge as described in §112.1(b) to relate information on the exact address or location and phone <br /> number of the facility; the date and time of the discharge, the type of material discharged; estimates of <br /> the total quantity discharged; estimates of the quantity discharged as described in §112.1(b); the source <br /> of the discharge; a description of all affected media, the cause of the discharge; any damages or injuries <br /> caused by the discharge, actions being used to stop, remove, and mitigate the effects of the discharge; <br /> whether an evacuation may be needed; and, the names of individuals and/or organizations who have <br /> also been contacted. Immediately update the SPCC plan to include this information. <br /> 26. The SPCC plan does not organize portions of the plan describing procedures you will use when a <br /> discharge occurs in a way that will make them readily usable in an emergency. Immediately update the <br /> SPCC plan to include this information. <br /> 31. Daily inspection records for 2011 were not available for review. Inspections and testing shall be <br /> conducted on all above ground liquid petroleum containers and tanks 55 gallons and larger, as specified <br /> in the SPCC plan. Records of these inspections and tests shall be signed by the appropriate supervisor <br /> or inspector and kept on site with the SPCC Plan for a period of three years. Immediately begin <br /> necessary testing and inspections for all APSA regulated containers and maintain a record on site with <br /> the SPCC Plan. <br /> 32. Annual training is not being conducted for the above ground petroleum storage program and <br /> containers. At a minimum, oil-handling personnel must be trained in the operation and maintenance of <br /> equipment to prevent discharges, discharge procedure protocols, applicable pollution control laws, rules <br /> & regulations, general facility operations, and the contents of the facility's SPCC Plan. Annual briefings <br /> must be conducted to assure adequate understanding of the SPCC Plan and highlight and describe any <br /> known discharges or failures, malfunctioning components, and any recently developed precautionary <br /> measures. Immediately conduct necessary training for all oil handling personnel.
