The contact list did not include phone numbers for San Joaquin County Office of Emergency Services (OES), San Joaquin County Environmental Health Department (EHD), or the cleanup contractor for this facility. The phone number on file with the facility for OES was outdated. No facility response coordinator was listed in the Plan; instead the phone number for the facility response coordinator was listed under the title "emergency response contractor." The California Office of Emergency Services has been changed to the California Emergency Management Agency (CaIEMA). The Plan must include a contact list and phone numbers for the facility response coordinator, National Response Center, cleanup contractors with whom you have an agreement for response, and all appropriate federal, state, and local agencies who must be contacted in case of a discharge. Immediately update the contact list for this facility to include all of the required phone numbers. Complete & submit a copy of the Return to Compliance Certification to the EHD by November 21, 2010 along with any supporting paperwork and a statement documenting the corrective actions that have been or will be taken for each violation. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
