Fred Mc Phearson <br />Page 2 <br />Three copies of the draft FRP and the "Executive Summary" are to <br />be submitted to PHS/EHD. One copy will be made available for <br />public review at the PHS/EHD office during normal business hours, <br />the second copy will be placed at the Stockton Public Library for <br />public review, and the third copy will be for PHS/EHD staff <br />review. <br />Please be advised that pursuant to Section 2652(d) of the <br />Underground Storage Tank Regulations (California Code of <br />Regulations, Title 23, chapter 16) a quarterly monitoring report <br />shall be submitted to Public Health Services, Environmental <br />Health Division and Central Valley Regional Water Quality Control <br />Board (CVRWQCB) until the investigation and cleanup of the site <br />is deemed adequate. Quarterly sampling and reporting is to begin <br />immediately. The report is to show the: <br />-status of investigation and cleanup activities <br />including the results of all investigations completed <br />to date; <br />-proposed continuing or next phase of investigation; <br />-method of cleanup proposed or implemented, schedules <br />for the completion of the investigation of the site <br />and remediation; <br />-method and location of disposal of the released <br />hazardous substance and any contaminated soil, <br />groundwater or surface water; <br />-manifest required for transport of hazardous <br />substances; <br />-monitoring well data including groundwater elevation, <br />pH, conductivity, gradient; <br />-analytical results of groundwater or soil sampling; <br />and <br />-site map showing the "zero line" of contamination, if <br />known, and changes in the above measurements over the <br />last quarter. <br />If you have any questions contact Margaret Lagorio, Senior RENS, <br />at (209) 468-3449. <br />Jogi Khanna, M.D., M.P.H. <br />Ilth Offic r <br />Laurie A.
