Well Destruction Work Plan <br />Facility Site Code: 19093 <br />February 20, 2012 <br />Page 2 of 4 <br />L.. PRE -FIELD ACTIVITIES <br />Upon receipt of SJCEHD approval of this Work Plan, a Boring Permit application will be <br />completed and submitted to SJCEHD. The SJCEHD Boring Permit will be available on site <br />L" during the field activities. PSC will also coordinate schedules with the third parry utility, drilling, <br />and surface restoration subcontractors to perform the various related tasks. Upon receipt of the <br />t Boring Permit, PSC will coordinate the field schedule with subcontractors and Earthgrains for site <br />&. access to perform the well destruction activities. <br />PSC will prepare a Health and Safety Plan (HASP) for PSC personnel and their subcontractors <br />performing in -field activities. The HASP will include a brief summary of the scope of work, all <br />emergency contact information, and standard operating procedures for work to be performed. <br />The HASP will also include job safety analysis (JSAs) and all field safety forms necessary for the <br />scope of work. <br />PSC will mark the well locations and indicate total distances for utility clearance in white paint <br />on the street or sidewalk in front of the subject property for the USA state call-in notification. <br />PSC will call USA to schedule the utility locate upon approval of this Work Plan and receipt of <br />the approved permits. The SJCEHD will be notified at least 72 -hours prior to starting the field <br />activities. <br />IN -FIELD ACTIVITIES <br />During in -field activities, safety meetings will be held prior to starting field activities each day. <br />All PSC and subcontractor personnel will attend the meetings or be provided copies of the HASP <br />and appropriate JSAs when they arrive on site. All personnel will be informed of the health and <br />I— safety concerns regarding the work to be performed and the site conditions. The HASP and JSAs <br />will be signed before commencing and after completion of work on the site each day. <br />Subdynamic Utility Detection Services, Inc. (SubDynamic) will perform the third party utihty <br />location clearance prior to starting the field activities. Subdynamic will meet with PSC and the <br />• drilling personnel on the morning that the field activities will commence. All essential utilities <br />will be identified within the specific drilling locations and drilling will not commence until <br />a' clearance has been assured by SubDynamic. Since the borehole locations are existing wells, the <br />additional clearance activities of hand angering or using an air knife are not required. If wells are <br />located in close proximity to subsurface utilities, an air knife or hand auger may be required or <br />�- alternatively may be pressure grouted in place. <br />The concrete and/or asphalt surface may need to be pre -cored in preparation for the well <br />destruction borings. Osborne Concrete Coring will perform the coring and/or saw cutting <br />activities. Gregg Drilling of Martinez, California, a C57 -licensed contractor, will perform the <br />well destruction activities using a hollow -stem auger drill rig. All drilling equipment and tools <br />shall be decontaminated prior to beginning the field activities. All reusable equipment shall be <br />thoroughly washed with a Liquinox (or equivalent) solution and/or a heated pressure washer. <br />Wells in which laboratory data indYate no presenc/of conpminaV of concern will be destroyed <br />�- <br />first. Terse wells include; MW -2, MW -3, MW -5, MW -6, MW- , MW -9, MW -10, MW -11, SP - <br />3, SP -4, and St -5.
