(ECM) is <br />submitting the enclosed boring permit application for the ISCO Injection Project at the California <br />Army National Guard Field Maintenance Shop #24 located in Stockton, California. ECM has <br />included the following as part of the application package: <br />• Completed San Joaquin County — Environmental Health Department (SJEHD) Boring <br />Permit Application, <br />• Completed Master File Record Information (MFR) form <br />• Brief work scope summary <br />• Boring plan, <br />• Email approval of the Work Plan from the Central Valley Regional Water Quality Control <br />Board (CVRWQCB), and <br />• Check for $500.00 to cover the Boring Permit Fee ($125) and Inspection Fees ($375). <br />Per our discussion, we will be completing 36 borings during two (2) mobilizations. The first <br />mobilization is scheduled during the week of July 14, 2014, and the second during the week of <br />August 11, 2014.
