APP <br />Well Installation and Sampling at California National Guard Field Maintenance Shop <br />Stockton, California <br />2.2.2 Description of California National Guard Field Maintenance Shop in <br />Stockton, CA <br />The FMS is located in one of four divisional areas at 8020 South Airport Way in Stockton, <br />California (See Figure 1). The divisional areas comprise of the Army Aviation and Support <br />Facility, the Combined Support and Maintenance Shop, the Field Maintenance Shop (FMS) and <br />the Armory. <br />The FMS comprises the following: a one-story maintenance building composed of work bays, <br />supply areas and offices; a vehicle inspection rack, various hazardous materials sheds; a <br />concrete wash pad with associated holding tank. <br />The potential contaminants found in the soil are Total Petroleum Hydrocarbons as Diesel <br />(TPH-d), Total Petroleum Hydrocarbons as Gasoline (TPH-g), Benzene, Toluene, <br />Ethylbenzene, Xylenes and Total Lead. <br />Figure 2 shows the project monitoring well locations at the FMS in Stockton, CA. <br />2.3 CONTRACTOR ACCIDENT EXPERIENCE <br />The OTIE Environmental Health and Safety (EHS) Performance Summary, including <br />Experience Modification Rate (EMR); injury and illness incident rates; Days Away, Restricted, <br />Transfer (DART) incident rates; etc., is included in Appendix A of this APP. Also included is the <br />OTIE 2008 Occupational Safety and Health Administration (OSHA) Form 300A Summary of <br />Work -Related Injuries and Illnesses, and the OTIE corporate trend analyses. <br />2.4 LISTING OF PHASES OF WORK AND HAZARDOUS ACTIVITIES REQUIRING ACTIVITY <br />HAZARD ANALYSES <br />It is recognized that certain work activities are inherently more hazardous than others. For these <br />operations, a detailed activity hazard analysis (AHA) will be conducted for each associated work <br />task that specifies corrective actions to be taken to control the hazard. AHAs will be produced, <br />reviewed, and modified, as necessary, to address new work activities and changing site <br />conditions as the work progresses.
