APP <br />Well Installation and Sampling at California National Guard Field Maintenance Shop <br />Stockton, California <br />4.3 CORPORATE SAFETY AND HEALTH MANAGER <br />William S. Fink, CIH, CSP, CHMM is the SHM for the project. He is responsible for the <br />development, implementation, oversight, and enforcement of this APP/SSHP. He is available to <br />provide consultation, as needed, on health and safety issues that may arise during the project <br />and to ensure the APP/SSHP is fully implemented. The SHM will coordinate any modifications <br />to the safety documents with the PM and the Contracting Officer (CO) and approve <br />APP/SSHP/AHA amendments for specific site tasks. The SHM will also review accident reports <br />and results of daily inspections as well as coordinate policy or procedure changes that may <br />impact the project with the PM. <br />4.4 SITE SAFETY AND HEALTH OFFICER <br />Mr. Lik-See Chung is the SSHO for this project. He will assist and represent the OTIE SHM in <br />on-site training and the day-to-day on site implementation and enforcement of the accepted <br />APP/SSHP. He will be assigned to the site as work proceeds. Mr. Chung will conduct initial site- <br />specific training and has authority to ensure site compliance with the following: <br />• Specified safety and health requirements including Federal and State OSHA <br />regulations. <br />• Enforcement of all aspects of the APP/SSHP including, but not limited to: <br />• The activity hazard analyses <br />• The use of PPE <br />• Decontamination procedures <br />• Site control <br />0 <br />The SSHO has authority to stop work if unacceptable safety or health conditions exist and take <br />necessary action to reestablish and maintain safe working conditions. The SSHO will consult <br />with and coordinate any modifications to the APP/SSHP/ANAs with the Corporate SHM, the <br />FTL, the PM, and the CO. The SSHO will serve as a member of the Contractor's quality control <br />staff on matters relating to safety and health. The SSHO will conduct accident investigations <br />and prepare accident reports. The SSHO will review results of daily quality control inspections <br />and document safety and health findings in the daily reports. In coordination with site <br />management and the SHM, the SSHO will recommend corrective actions for identified <br />deficiencies and oversee the corrective actions. The SSHO will provide continued support for <br />upgrading/downgrading the level of personal protection. The SSHO will be responsible for <br />evaluating air monitoring data and recommending changes to engineering controls, work <br />practices, and PPE. <br />4.5 EMERGENCY RESPONSE COORDINATOR <br />Mr. Chung will serve as the emergency response coordinator for the site during work activities. <br />He will coordinate with local emergency response organizations prior and during work activities <br />as needed. <br />OTI E <br />
